胃管旁置小管协助吸痰方法探讨

摘　　要：目的探讨昏迷或各种吞咽障碍留置胃管患者应用胃管旁置小管协助吸痰,对预防肺部感染及使用吸痰器次数的影响。方法随机将79例昏迷或各种吞咽障碍留置胃管的患者分为实组组和对照组,每组分别为40例和39例。实验组应用常规吸痰加胃管旁留置小管吸痰,对照组只应用常规吸痰,比较两组患者肺部感染及使用吸痰器次数的差异。结果实验组肺部感染发生率20.0%,对照组为30.77%,两组比较,差异有统计学意义(P<0.01);实验组平均每天使用吸痰器吸痰次数为4.0±1.85次,而对照组则为14.0±1.75次,两组比较,差异有统计学意义(P<0.01)。结论昏迷或各种吞咽障碍留置胃管患者应用胃管旁置小管协助吸痰,是预防肺部感染,减少吸痰器使用次数,减少患者痛苦,减轻护士的工作量是一种有效的方法。Objective To explore the effect of aspirating sputum assisted with small sputum-aspirating tubes beside the indwelling gastric tube on preventing pulmonary infection and reducing the frequency of sputum aspirator application. Methods Seventy-nine patients with coma or deglutition disorder were randomly allocated to the control group and the experiment group with 39 cases and 40 cases in each group separately. Forty cases in the experiment group were used with conventional aspirating sputum assisted with small sputum-aspirating tubes beside the indwelling gastric tube, while in the control group, only conventional aspirating sputum was used. The rate of pulmonary infection and frequency of sputum aspirator application were compared between the two groups. Results The rate of pulmonary infection was significantly lower in the experiment group than that in the control group (20.0% vs 30.77%, P < 0.05). The frequency of using sputum aspirator in the experiment group was significantly lower than that in the control group (4.0±1.85 vs 14.0±1.75, P < 0.01). Conclusions The clinical application of aspirating sputum with small sputum-aspirating tubes beside the indwelling gastric tube was an effective method in preventing the pulmonary infection, reducing the frequency of sputum aspirator application, lessoning the patients' suffering and decreasing the nurses' workload.
