摘  要:目的 :探讨支气管哮喘患者 CD8+ T细胞对单核 /巨噬细胞抗原递呈功能的影响。方法 :哮喘患者 2 0例 ,健康对照 2 2例 ,分别取静脉血 5 m L,并分离 MΦ、CD8+ T细胞和 B细胞。每份血样分成 4组 :MΦ递呈抗原组、CD8+ T细胞参与 MΦ递呈抗原组、 CD8+ T细胞体外活化后对 MΦ递呈抗原影响组及自然状态下 CD8+ T细胞对MΦ细胞递呈抗原影响组。各组用 CTL L2 P抗原刺激 18h后 ,洗去刺激原 ,与自体 B细胞共同孵育 10 d,吸取上清液 ,测定特异性 Ig M、 Ig E、 Ig G含量。结果 :1哮喘患者 MΦ单独递呈抗原 ,自体 B细胞特异性 Ig M (A4 90值 ) (0 .0 34± 0 .0 2 2 )明显低于健康人 (0 .116± 0 .0 80 ) (P<0 .0 5 ) ;CD8+ T细胞与 MΦ 共同培养递呈抗原时 ,产生的特异性 Ig M(A4 90 值 ) (0 .0 31± 0 .0 2 1)低于健康人 (0 .0 79± 0 .0 6 4 ) (P<0 .0 5 ) ;2哮喘患者 CD8+ T细胞与MΦ 共同培养递呈抗原时 ,产生的特异性 Ig G (A4 90 值 ) (0 .10 2± 0 .0 4 1)明显高于健康人 (0 .0 81± 0 .0 6 7) (P<0 .0 5 ) ,自然状态下及体外活化后 CD8+ T细胞与递呈抗原 MΦ共同培养 ,产生的特异性 Ig G (A4 90 值 ) (0 .10 5±0 .0 6 6 ,0 .0 79± 0 .0 5 9)与健康人 (0 .0 6 6± 0 .0 38,0 .0 6 9± 0 .0 4 7)Objective To study the effects of CD8 + T lymphocytes on processing macrophages (MΦ) of patients with asthma. Methods The MΦ and CD8 + T lymphocytes were isolated from peripheral blood of 20 patients with asthma and 22 health donors, respectively. Every sample was divided into four groups. All groups were stimulated with CTLL 2P antigen for 18 hours and CTLL 2P antigen were washed away, then they were cultured with B lymphocytes together. The levels of secreted CTLL 2P-specific IgM, IgG, and IgE in the supernatant were measured after 10 days. Results ①When MΦ processed antigen only, the level of CTLL 2P-specific IgM of patients with asthma(0.034±0.022) was lower than that of health donors (0.116±0.080) (P<0.05). The level of CTLL 2P-specific IgM of patients with asthma (0.031±0.021) was lower than that of health donors (0.079±0.064) (P<0.05),when CD8 + T lymphocytes were cultured with mononuclear phagocytes together. ②When CD8 + T lymphocytes were cultured with mononuclear phagocytes together, the level of CTLL 2P-specific IgG of patients with asthma (0.102±0.041) was significantly higher than that of health donors (0.081±0.067) (P<0.05). However, the IgG levels had not differences when active CD8 + T lymphocytes or natural CD8 + T lymphocytes were cultured with antigen processing mononuclear phagocytes (0.105±0.066,0.079±0.059),compared with health donors (0.066±0.038,0.069±0.047)(P>0.05). ③The CTLL 2P-specific IgE level of patients with asthma (0.171±0.154) was higher than that of health donors (0.147±0.059) (P<0.05), when CD8 + T lymphocytes were cultured with mononuclear phagocytes together to process antigen. Conclusion The CD8 + T lymphocytes of patients with asthma have different regulatory effect on immunolglobulin production from antigen processed by MΦ.
