颈黄韧带对颈黄韧带叠压症的临床和组织学研究

摘　　要：目的研究颈黄韧带对颈黄韧带叠压症的临床影响和组织病理学特点。方法对1994～2003年经后路颈椎管扩大成形术后随访的76例黄韧带叠压症进行临床观察和黄韧带进行光镜和透射电镜组织病理学观察,并与正常颈椎黄韧带进行比较。结果切除黄韧带和扩大椎管成形术后76例临床症状显著改善,光镜和电镜发现患者组黄韧带比正常组弹力纤维明显减少,胶原纤维代偿性增多,纤维排列紊乱,近椎板附丽部明显变性和骨化,比较两组有显著意义(P<005)。结论退变和骨化的黄韧带变长变薄,缺乏弹性和柔韧性,导致椎管有效容积下降压迫脊髓,是颈黄韧带叠压症的主要原因。Objective:To sudy the influence of clinic and pathology histology of folding compression of cervical ligamentum flavum Method:Since 1994 to 2003,the clinical manifestation and laminoplasty treatment of 76 patients with folding compression of cervical ligamentum flavum were discribed They were treated by enlarged laminoplasty of cervical spinal canal The microstructures and ultrastructures of the cervical ligamentum flavum were observed in 13 normal adults and 76 patients with folding compression of cervical ligamentum flavum The structure menifestation were compared with each other in two groups Result:The 76 patients of removed ligamentum flavum in enlargement and laminoplasty,their clinical menifastation were significant improved In the patient’s group,cervical ligamentum flavum showed decrease of the elastic fibers and proliferation of the collagen fibers,which run in random fashion,with marked fibrotic changes as well as calsification in attach the partital of the laminum There were significant difference between the two groups( P<0 05 ) Conclusion:The cervical ligamentum flavum with folding compression of cervical ligamentum flavum shows degeneration and calcification in a few of cases The former would destroy the elasticity and the pliability and toughness would be decerase the effective volum of cervical spinal canal and would be main cause folding compression of cervical ligamentum flavum
