关节镜微创技术在关节外的应用与疗效

摘　　要：目的探讨关节镜微创技术在关节外的临床应用与疗效。方法关节镜监视下行关节外手术134例,其中臀肌挛缩射频汽化松解术38例,腘窝囊肿摘除25例,腕横韧带切开松解治疗腕管综合征22例,关节内骨折18例,网球肘8例,跟腱炎4例,髌腱炎3例,足踝关节融合8例,钢板螺钉取出5例,胸锁乳突肌切断治疗斜颈3例。结果臀肌挛缩术后弹响消失,下肢交腿试验阴性、并膝下蹲功能正常;腘窝囊肿术后无复发及血管神经损伤;腕横韧带切开术后感觉运动功能正常;关节镜下撬拨达解剖复位,关节功能良好;肌腱末端病术后疼痛解除12例,症状明显缓解3例。无感染、血管和神经损伤等并发症。结论关节镜微创技术在关节外的应用扩大了关节镜技术领域,具有微创、操作简便、安全可靠的优点。Objective:To explore the new technique application of arthroscopy assisted in exterior joint surgery Method:One hundred and thirty four procedures were done under the arthroscopy in the exterior of joint In this group gluteus contracture releasing by radiofrequency in 38,percutaneous reduction by leverage and internal fixation treating the fracture under the arthroscopy in 18,removing of the screw and plate of the femoral shaft in 5,removing of popliteal fossa cyst in 25,the carpal tunnel syndrome releasing in 22 Topaz with arthroscopically assisted for treatment of Achilles tendonitis in 4,tennis elbow in 8,and patellar tendonitis in 3 Radiofrequency releasing sternocleidomastoideus muscle contracture for wry neck in 3,foot and ankle arthrodesis in 8 cases Result:The function was satisfactory after arthroscopical treatment assisted for all procedure The symptoms were disappeared after operation No blood vessel and nerve injury,no infection occurred Conclusion:Arthroscopically assisted new technique not only for joint disease but selective for the exterior joint surgery The procedure shows minimum invasion,less pain and easy for early rehabilitation,the result is satisfactory
