内皮素在海洛因依赖早期肾损害中的作用

摘　　要：目的 :探讨血浆内皮素 (ET - 1)在海洛因依赖者早期肾损害中的作用。方法 :采用竞争酶免疫方法检测 80例海洛因依赖者 (吸毒组 ) ,6 1名健康人 (正常对照组 )的血浆ET - 1水平 ,用免疫散射比浊法检测尿 β2 微球蛋白(β2 -m)、尿白蛋白 (Alb)、尿α1 微球蛋白 (α1 -m)。结果 :海洛因依赖者血浆ET - 1水平显著高于正常对照组 (P <0 0 1) ;且血浆ET - 1与尿 β2 -m ,α1 -m ,Alb ,吸毒时间 ,吸毒剂量呈正相关 (P <0 0 5 ,P <0 0 0 1,P <0 0 1,P <0 0 0 1,P <0 0 5 ) ,与内生肌酐清除率 (Ccr)呈负相关 (P <0 0 1)。结论 :ET - 1参与介导了海洛因依赖早期肾损害的发病过程 ,ET - 1是预测海洛因依赖早期肾损害的一个重要指标。Objective: To investigate the effect of plasma endothelin-1(ET-1) in early stage renal impairment of heroin addicts. Methods: The ET-1 of heroin addicts group (80 cases) and normal control group(61 cases) was detected with competitive enzyme immunoassay, and the β 2-microglobulin(β 2-m), microalbminuria(Alb), α 1 microglobulin(α 1-m) were determined with immunoassay dispersion corrupted methods. Results: ET-1 in heroin addicts was significantly higher than that in the control group(P<0.01), and ET-1 was evidently correlated with β 2-microglobulin(β 2-m), microalbminuria(Alb), α 1 microglobulin(α 1-m), the duration, dosage (P<0.05, P<0.001, P<0.01, P<0.001, P<0.05), and negatively correlated with creatinine clearance rate(P<0.01). Conclusion: ET-1 might take part in the progress in early stage renal impairment of heroin addicts. ET-1 is an important index to prognosticate the early stage renal impairment of heroin addicts.
