经食管三维超声心动图的临床应用

摘　　要：报告经食管三维超声心动图(3D-TEE)59例临床应用的体会。此检查所获图像可分三个区域:右区可显示上腔静脉、右房、房间隔与左房等,并能观察房间隔缺损的位置、大小、形状与类型。中区显示两条大动脉的起源与走向,右室流出道狭窄者见流出道有环形嵴状凸起,呈现出葫芦状的双腔右室。左区能显示整个左室、左房的大小,二尖瓣的形态与对合情况,二尖瓣脱垂者可见叶瓣游离缘处某一区域向左房脱出。观察表明:3D-TEE轮廓清晰,形象直观,有利于解剖结构的辨识和空间方位的理解,对多种心脏疾病的诊断有一定帮助。In this paper the authors reported the clinical application of three-dimensional transesophageal echocardiography (3D TEE) in 59 patients. The 3D TEE images were divided into three areas. The right area includes superior vena.cava, right atrium,interatrial septum and left a-trium. In the middle area,the origin and course of the two great arteries could be visualized. In left area , the contour and size of the left ventricle and left atrium and the shape and coaptation of the mitral valve could be visualized. Therefore, the reconstructed 3D TEE allow identification of the cardiac anatomic structures and understanding of the spatial relations. It can be used not only to observe the left ventricular shape, but also to visualize the atrial septal defects, left atrial myxo-ma, obstruction of the right ventricular outflow tract, aneurysm of sinus of Valsalva, mitral valve prolapse, mitral regurgitation and mitral stenosis.
