摘 要:目的:探讨供受体及其自体肺缺血再灌注损伤时组织结构变化的特点及规律。方法:供受体各6只健康成年犬,用4C改进EC液单次肺动脉灌注获取供肺,受体行一侧全肺切除后植入移植肺。分组:供体正常肺(A)、受体正常肺(B)、移植肺(C)、受体自体肺(D)。测试、观察各组肺含水量、肺泡体积密度(Vv)、出血分级及形态变化。 结果:C、D与A组比较C组有非常显著性意义(P<0.01),D组有显著性意义(P<0.05);C、D与B组比较aVv有显著性意义(P<0.05);C、D与A组比较出血分级有非常显著性意义(P<0.01)。结论:肺移植缺血再灌注时受体自体肺与移植肺均有损伤,前者较轻;Vv可作为评价肺损伤程度的客观指标。Objective: To investigate the structural changes of the donor and recipient as well as native lung in terms of the recipient lung being injured by ichemia-reperfusion. Methods: Twelve dogs were anesthetized and the donor were rooted in pulmonary perfusion with sing flush and improved Euro-Collins solution(iECS) at 4 C.The recipient was performed unilateral pulmonary pneumonectomy before the donor lung was transplanted.Four groups lung sample were harvested (normal donor lung=A, normal recipient lung =B,transplanted lung =C and native lung=D) respectively,after which tissue samples of the lung were taken for the volume density of alveolar(aVv),lung liquid volume(LLV),morphological obserservation and a graded scale of haemorrhage. Results: The dogs in C and D group showed an increase in the LLV(P<0.01)and lowered in the aVv(P<0.05)compared with the dogs in A and B group respectively,then showing significant difference(P<0.01) in the graded scale of haemorrhage. Conclusion: The native and transplanted lung in unilateral lung allotransplantation model were both injured by ischemia-reperfusion,but the native lung had a little damage than that of the transplanted lung, Vv, however, may evaluate to pulmonary degree of injury.
