摘　　要：目的 :探讨胎盘异铁蛋白在异常妊娠诊断中的应用价值。方法 :EL ISA法前瞻性的检测 2 0 0例最初正常的孕妇 ( 12～ 15周 )血清中的 PL F,并随访妊娠结果 ,用 ROC曲线评价值 PL F在异常妊娠诊断中的应用价值。结果 :1自然流产组、妊娠高血压综合征组、早产组和低体重新生儿组血清中 PL F含量均明显低于正常妊娠组 ,且存在统计学意义的差异 ( P<0 .0 5 ) ,更进一步将低体重新生儿组新生儿的体重与 PL F含量作直线相关分析 ,发现两者有明显的正相关 (γ=0 .873,P<0 .0 5 ) ;2 PL F预测异常妊娠的实验室诊断参数 :敏感性、特异性、阳性预测值、阴性预测值及 ROC曲线下的面积 ( AUC)分别为 92 .4 %、80 .0 %、6 9.3%、95 .6 %和 0 .86。结论 :PL F与多种异常妊娠有着密切的关系 ,可用于孕妇孕期的监护 。Objective:To investigate the value of placental isoferritin in diagnosing pathological pregnancy.Methods:A prospective study was performed, The placental isoferritin in sera from 200 initial normal pregnant women (from 12 to 15 weeks) were detected by enzyme linked immuno absorbent assay(ELISA) and the outcomes of pregnancies were followed up to evaluate the value of placental isoferritin in diagnosing abnorm gestation used ROC curve.Results:①In maternal serum placental isoferritin level of normal pregnant women were higher than that in spontaneous abortion group, preeclampsia group, premature delivery group and low birth weight group, and there was a significantly difference(P<0.05 ),further analysis showed that there was a strong correlation between the concentration of placental isoferritin from low birth weight group and birth weigh(γ=0.873, P<0.05). ② At the cut point of 10 U/ml placental isoferitin, the sensitivity, specificity, positive predictive value, negative p redictive value and AUC were 92.4%, 80.0% ,69.3%,95.6% and 0.86, respectively.Conclusions:The decrease of serum placental isoferritin was associated with abnorm gestation and it could serve as a predictive marker to identify women at risk of abnorm gestation.
