分节段整骨手法治疗神经根型颈椎病的临床研究

摘　　要：目的 :观察研究根据颈椎不同节段运用相应的调整骨关节、椎体的手法治疗神经根型颈椎病的临床疗效。方法 :将 1 84例患者随机分为观察组 (分节段整骨手法 )、对照组 1 (侧旋提推法 )、对照组 2 (旋转复位法 )、对照组 3 (软组织按摩 ) ,每组 46例进行治疗 ;分别统计比较治疗前后的症状、体征评分 ,颈椎生理曲度测量值 ,疗效变化等等。结果 :观察组可明显改善患者的症状体征 ,与对照组 1、2、3比较有显著差异 (P<0 .0 5 ;P<0 .0 5 ;P<0 .0 1 ) ;观察组、对照组 1对颈椎生理曲度测量值的改善与治疗前比较有显著差异 (P<0 .0 1 )。结论 :在治疗神经根型颈椎病中 ,根据不同节段选用不同的调整椎体、关节的手法 ,比单纯运用一种调整手法或纯粹的软组织按摩方法疗效明显。Objective:To observe the clinical effects for nerve-root-type cervical spondylosis according to the regulating manipulation for joints and vertebrae base on the different section of cervical vertebrae.Methods:184 cases were divided into observation group (manipulation according to sections),control group Ⅰ (side-twist and lifting-pushing reduction manipulation), control group Ⅱ (rotating manipulation), control group Ⅲ (soft tissue massage) randomly.Every group got 46 cases.Separately statistical analysis and compare the difference of syndromes,score of physical sign,measured value of cervical spine physio-curve,effects according to treatments.Results:Observation group could obviously improve the syndrome and physical sign,compare with control group Ⅰ、Ⅱ、Ⅲ got typically difference (P<0.05,P<0.05,P<0.01);observation group and control groyp Ⅰ got obviously difference according to measured value of cervical spine physio-curve (P<0.01) in comparison with before cure. Conclusions:In clinc in treating nerve-root-type cervical spondylosis,according to different section of vertebrae to choose different manipulation technique of vertebrae and joints could get much better effects comparing with choosing single manipulation technique or simple soft tiss massage.
