经食道内镜超声检查在纵隔和肺疾病诊断中的作用

摘　　要：目的　评价内镜超声 (EUS)检查在纵隔和肺疾病诊断中的应用价值。方法　采用PENTAXFG 3 6UX内镜经食道超声检查纵隔疾病和肺癌 3 0例。结果　对 11例纵隔疾病EUS检查纵隔疾病的准确率为 81.8% ,内镜超声引导下细针吸取细胞学检查 (EUS FNA ) 5例 ,阳性率10 0 % ,准确率为 10 0 %。 19例肺癌患者EUS检查既了解病变纵隔侵润情况、纵隔病变与周围脏器的关系 ,又明确纵隔淋巴结转移数目 ,EUS检查肺癌纵隔侵润的准确率为 89.4% ,行EUS FNA 6例 ,阳性率 83 .3 % ,准确率为 10 0 %。结论　EUS是一项先进、安全、对纵隔和肺疾病的临床诊断有较大帮助的检查手段。Objective To evaluate the clinic value of endoscopic ultrasonography (EUS) in mediastinal disease and pulmonary disease.Methods PENTAXFG-36UX Type EUS was performed in 30 patients of mediastinal disease and pulmonary carcinoma.Results Examination of EUS in 11 patients with mediastinal disease.The accuracy of EUS was 81.8% in diagnosing mediastinal disease;the positive and the accuracy of Endosonography-guided fine needle aspiration (EUS-FNA) were 100% and 100%.Detection of EUS in 19 patients with pulmonary carcinoma could help to uderstand the condition of mediastinal invasion,the relation between mediastinum and nearby organs,and also play a part in clarify the metastasis of mediastinal lymph node.The accuracy of EUS was 89.4% in diagnosing pulmonary carcinoma with mediastinal invasion; the positive and the accuracy of EUS-FNA were 83.3 % and 100%.Conclusion EUS is an advanced, safe procedure that helps more with clinical diagnosis of mediastinal disease and pulmonary disease.
