家兔眼球后注射替曲朵辛的麻醉效果及毒副作用研究

摘　　要：目的 对替曲朵辛(TTX)溶液球后注射的麻醉效应、安全性和视网膜毒副作用进行初步评价。方法 健康成年家兔随机分组,每组6只,其中右眼球后注射不同剂量TTX及利多卡因溶液,左眼给予等量溶剂对照(柠檬酸钠缓冲液,pH4.3);分别采用角膜感觉试验、裂隙灯、视网膜电图、光电镜检查进行观察。结果 球后注射10μg TTX可致家兔全部死亡;≤5μg剂量家兔全部存活;具有明显的局部麻醉作用;5μg剂量组可观察到轻微视网膜毒性作用;而≤2.5μg则未见眼部毒性反应。 结论 球后注射低剂量TTX对家兔角膜可产生长效局部麻醉作用且无明显眼毒性反应,有希望应用于治疗某些疼痛性眼病及术中镇痛。Objective To determine the anesthetic effectiveness of Tetrodotoxin (TTX) and its toxicity to retma after its retrobulbar injection. Methods Health rabbits were assigned to six groups and 6 rabbits for each group. 50 μL of TTX(0. 5 μg, 1.0 μg,2. 5 μg,5 μg or 10 μg)were injected retrobulbarly into the right eyes of rabbits in 5 experimental groups respectively and 5 μL of 2% lidocaine for the other group, and the left eye of each rabbit received 50 μL of pH 4. 3 sodium citrate vehicle as a control. Corneal sensation was tested after administration of drugs. Slit-lamp examination at 12 and 24 hours after administration, electroretinography(ERG) before and 24 hours after administration and light electron microscopy examination after 1 week were performed to detect drug toxicity to ocular tissues. Results A long-acting topical anesthetic effect was demonstrated at a dose of 5 μg TTX,and the lasting-time was from 0. 37 to 5. 7 hours in 0. 5 to 5 μg of TTX. Shorter-acting anesthetic action up to 0. 34 hours was found in lidocaine-injected rabbits. 10 μg of TTX resulted in rabbit death due to systemic toxicity. Decrease of ERG b-wave and morphologic changes were seen in 5. 0 μg TTX group and no these changes in 2. 5 μg TTX group. Conclusion TTX is an effective,long-lasting topical anesthetic agent for ocular pain control,and less 2. 5 μg of TTX is a safe dose to topical application.
