胰腺外伤51例临床分析

摘　　要：目的 :探讨如何对胰腺外伤进行准确的诊断和有效的治疗。 方法 :回顾分析过去 10年间所收治的胰腺外伤 5 1例的临床诊断及治疗经过。 结果 :术前 4 5例行胰腺CT检查 ,均有阳性发现 ;4 5例行B型超声检查 ,18例有阳性发现 ,阳性率4 0 % ,两者阳性率差异明显 (P <0 0 1)。血淀粉酶明显升高者 9例。非手术治疗 3例 ,均治愈 ;手术治疗 4 8例 ,术后 2 4h死亡 6例 ,均为有严重合并伤的病例。 结论 :从影像学的诊断上来看 ,CT明显优于B超 ,胰腺CT可以作为胰腺外伤的确诊依据 ,胰腺外伤的治疗应以手术为主。Objective To explore an accurate diagnosis and effective treatment for pancreas trauma Methods Retrospective analysis of 51 patients suffered from pancreas injuries from January 1993 to December 2001,including the clinical diagnosis and treatment processes was made.Results Before the operation,45 patients had computed tomography(CT)scan,all had positive results,the positive rate was 100%,while 45 cases had B-Ultrasonic examination,among which 18 persons had positive results,the positive rate was 40%.Nine cases had high blood level of amylase;48 patients had operations,of whom 6 with severe compound trauma died 24 hours after the operation. Conclusion Most patients with pancreas uinjuries having compound trauma must be immediately operated so to get a definite diagnosis and prompt treatment.The diagnosis of pancreas trauma should be on the base of CT manifestation before the operation.
