壮肝逐瘀煎治疗慢性病毒性乙型肝炎肝纤维化临床研究

摘　　要：目的 :观察壮肝逐瘀煎对慢性病毒性乙型肝炎肝纤维化患者的疗效。方法 :将120例患者按完全随机非盲法分为两组 ,观察组给予口服壮肝逐瘀煎免煎中药颗粒 ,每日1剂 ,以200ml开水冲后分2次温服 ;对照组给予口服大黄虫丸 ,每次6g ,每日2次。两组根据不同病情加用复合维生素B ,维生素C或强力宁等。3个月为1疗程 ,两组均观察1个疗程。治疗前、后检测血清透明质酸(HA)、层粘蛋白(LN)、Ⅳ型胶原(ⅣC)、HBV血清标志物及肝功能。结果 :观察组疗效优于对照组(P<0.05);两组治疗前、后HA ,LN ,ⅣC明显下降(P<0.05),而观察组下降更显著(P<0.01) ,观察组与对照组比较有显著性差异(P<0.05) ;观察组HBV血清标志物改善也较对照 组明显(P<0.05)。结论 :采用壮肝逐瘀煎治疗慢性病毒性乙型肝炎肝纤维化有较好的疗效。Objective:To observe the curative effects of Zhuanggan Zhuyu Granule on the fibrosis of liver due to chronic virus B hepatitis.Methods:120cases were randomly divided into two groups.The treatment group was given Zhuanggan Zhuyu Granule by oral administration with1bag for two times/day,after being dissolved in200ml boiling water;the control group was treated with Dahuang Zhechong Bolus(6g/2times/day).Based on the patient's condition,Vitamin B complex,Vitamin C and compound ammonium glycyrrhetate injection were supplemented for both groups.The treatment course lasted3months.Then,the serum HA(hyaluronidase),LN(laminin),IV_C(type IV collagen),HBV and liver function were detected before and after the treatment.Results:The total effective rate in the treatment group was better than that of the control group(P<0.05).The HA,LN,and IV_C were obviously lowered(P<0.05),especially in the treatment group(P<0.01);the indexes of liver function were also improved with the significant differences between the two groups(P<0.05),and the HBV in treatment group was improved more distinctively than that in the control group(P<0.05).Conclusion:Better curative effects can be achieved by treating the fibrosis of liver due to chronic virus B hepatitis with Zhuanggan Zhuyu Granule.
