摘  要:目的 :研究高血压患者左室构型的改变。方法 :采用彩色超声心动图对 2 2 6例原发性高血压与5 1例正常对照组进行心脏结构的测定。结果 :正常构型组 :左室内径、室壁厚度、LVMI、RWT保持正常 ,左房内径 (LAD)、升主动脉内径 (Aos)较对照组增宽。向心性重构组 :LVMI虽在正常范围 ,但明显高于对照组 ,RWT超过正常范围 ,左室壁厚度开始增厚并高于对照组 ,左室舒张末径 (LVEDD)小于对照组。向心性肥厚组 :左室舒张末期室间隔厚度 (IVSTD) ,左室舒张末期后壁厚度 (LVWPTD)最厚。离心性肥厚组 :IVSTD、LVWPTD增厚虽有所减轻 ,但LVMI、LVEDD最大。结论 :正常构型组、向心性重构组所占比例较大 。Objective:To study the changes of the left ventricular (LV) configuration in essential hypertension.Methods:Vsing colour echocardiography,the cardiac structures of 226 patients with essential hypertension and 51 controls were measured.Results:In group with normal configuration,the left venticular internal diameter (LVEDD),the diastolic inter ventricular septal thickness (IVSTD),LVMI and RWT was normal,but the left atrium internal diameter (LAD) and aorta ascendens internal diameter (AOS) were wider than that of controls.In group with concentric remodling (CCR),LVMI was normal,but higher than that of controls,RWT was over the normal level,the thickness of LV wall was thicker than that of controls,LVEDD was thinner than that of controls.In group with concentric hypertrophy (CCH),IVSTD,the diastolic posterior wall thickness of LV (LVPWTD) was the thickest in the four groups.In group with eccentric hypertrophy (ECH),the thickness of IVSTD and LVPWTD was lighten than that in CCH,but LVMI,LVEDD is the thickest.Conclusion:The proportion of NG and CCR is larger than others.Less development of LV configuration can reduce the incidence and death rate of the vascular disease induced by essential hypertension.
