青春期发育影响因素多元回归分析

摘　　要：目的　探讨影响青春期发育的因素。方法　调查首次遗精 /月经初潮时间 ,测定血清瘦素、睾酮 /雌二醇 (E2 )、促卵泡成熟激素 (FSH)、黄体生成激素 (LH)、胰岛素水平 ,采用多元Logistic回归模型 ,对已遗精或有月经的 10 7例健康儿童及未遗精或无月经的 10 7例性别、年龄匹配儿童进行对照研究。结果　单因素分析显示 ,男性影响因素为身高、体重、胸围、臀围、血清瘦素、睾酮、FSH、LH、胰岛素 ;女性影响因素为身高、体重、胸围、臀围、血清瘦素、E2 、FSH、LH。多因素分析显示 ,体重 (OR =1.171,P <0 .0 1)、睾酮 (OR =1.0 10 ,P <0 .0 5 )、FSH (OR =1.5 0 5 ,P <0 .0 1)、血清瘦素 (OR =1.36 8,P =0 .0 0 1)为男性独立影响因素 ;体重 (OR =1.32 8,P <0 .0 0 1)、FSH (OR =1.173,P <0 .0 5 )、LH (OR =1.12 3,P <0 .0 5 )、血清瘦素 (OR =1.345 ,P <0 .0 0 1)为女性独立影响因素。结论　体重、FSH、血清瘦素均为男、女儿童青春期发育的独立影响因素 ,睾酮对男性青春期发育的影响十分突出 ,E2 对女性早期青春期发育的影响有待进一步研究。Objective There are many influencing factors in puberty development of schoolchildren.This study analyzes the effect of the influencing factors on puberty development.Methods Initial spermatorrhea、menophania time and determine the levels of serum leptin, testosterone(Testo)、Estradiol(E 2), follicle stimulating hormone(FSH), luteinizing hormone(LH) and insulin.Case-controlled study was carried by multivariable logistic regression in puberty development of 107 pairs of schoolchildren with and without spermatorrhea、menarche.Results Single factor analysis shows the influencing factors of schoolboys on puberty development are body height and weight,chest circumference,breech circumference,the level of serum leptin,Testo,FSH,LH and insulin;as for schoolgirls, influencing factors are body height and weight, chest circumference,breech circumference, the level of serum leptin,E 2,FSH and LH.Multivariable logistic regression shows factors of body weigh (OR=1.171,P<0.01),Testo (OR=1.010,P< 0.05),FSH (OR=1.505,P<0.01) and serum leptin (OR=1.368,P=0.001) are the independence influencing factors on the puberty development of schoolboys; factors of body weigh (OR=1.368,P<0.001),FSH (OR=1.173,P<0.05),LH (OR=1.123,P<0.05) and serum leptin (OR=1.345,P<0.001) are the independence influencing factors on the puberty development of schoolgirls.Conclusions Body weigh,FSH and serum leptin are independence influencing factors on puberty development of schoolchildren.Testo is an outstanding influence on puberty development of schoolboys, but the effect of E 2 on early puberty of schoolgirls remains to be elucidated.
