功能支具在烧伤后畸形手功能恢复中的作用及其效果评价

摘　　要：目的:观察经系统康复后特别是使用支具、功能支具对烧伤后手畸形功能康复的临床疗效。方法:1992-01/2002-12对整复外科1880例烧伤后手畸形患者进行手法治疗,分别给予蜡疗、超声波治疗仪治疗、并辅以特定的中药处方以药离子导入方式治疗,重要的是对每位患者特制支具或功能支具,佩戴2周～8个月,并制订了严重烧伤手畸形治疗后的评估标准。结果:随访10年,随访189例患者,其中158例手畸形患者手功能恢复,每个手动作基本正确,外形明显改善。结论:烧伤后手畸形通过手术治疗同时必须通过3个月以上的综合康复治疗,其中功能支具的使用对于手功能恢复是必不可少的一环。AIM:To evaluate the clinical effect of functional splint in the functional recovery of postburn deformed hands after systemic rehabilitation,especially with usage of splint and functional splint. METHODS:From 1992 to 2002,1 880 cases of postburn deformed hands from Department of Plastic Surgery were performed with wax therapy,ultrasonoscope therapy and auxiliary treatment with prescription of Chinese medicine(confected by means of drug ion-introduction).Specially-made splint or functional splint was fixed on the patients for 2 weeks to 8 months.Criteria of functional evaluation for postburn hands were formulated. RESULTS:After 10-year follow-up,158 of 189 cases have recovered hand function with improved appearance and correct actions. CONCLUSION:Postburn deformed hands must be operated,auxiliary with over 3 months' general rehabilitative treatment,and functional splinting is indispensable for functional recovery of postburn hand after operation.
