摘 要:目的:检测烧伤患者血清纤维连接蛋白(Fn)结合明胶活性,并探讨其与烧伤面积、深度和感染等因素的关系。方法:采用ELISA法平行检测345例烧伤患者、32例瘢痕患者和37例正常人对照血清Fn结合明胶活性。结果:随着烧伤面积和深度的增加,Fn结合明胶活性降低,特重度烧伤患者Fn活性显著低(P<0.05),瘢痕患者Fn活性显著高于正常对照(P<0.05)。烧伤患者Fn结合明胶活性与年龄、性别和血型无关。感染及吸入性损伤患者血清Fn结合明胶活性下降。同一烧伤程度不同性别Fn活性在中度和特重度烧伤组男性显著高于女性(P<0.05),瘢痕患者Fn活性男性显著高于女性(P<0.05);特重度烧伤Fn活性女性显著低于正常对照(P<0.05),瘢痕患者Fn活性女性显著高于正常对照(P<0.05)。结论:Fn活性降低与烧伤程度、吸入性损伤和机体感染有关。瘢痕患者血清Fn活性增加可能与其发病机制有关。Fn活性检测与含量测定具有不同病理生理意义。AIM:To detect the activity of serum fibronectin(Fn) binding gelatin, and discuss the relationships between it and burned area, degree, infection,etc. METHODS: The activity of serum Fn binding gelatin in 345 patients with burns, 32 patients with scar and 37 cases of healthy individuals were detected parallelly with enzyme-linked immunoadsordent assay(ELISA). RESULTS: With the increase of burned area and degree, the Fn binding gelatin activity was decreased, the levels of Fn activity in most severely burned patients were obviously low(P< 0.05), while the levels of Fn activity in patients with scar obviously higher than that in the normal control group(P< 0.05). The Fn binding gelatin activity in patients with burns was independent of age, sex or blood type.The gelatin-binding activity of serum Fn was decreased in patients with post-burn infection and inbreathed injury. The Fn activity at middle and most severe burned degree in male was significantly higher than that in female in the same burned degree and different sex(P< 0.05), it was increased in male than that in female in patients with scar(P< 0.05). In most severe burned degree, the level of Fn activity in female was decreased,compared with that in the normal control group(P< 0.05), while in patients with scar, the level of Fn activity in female was increased,compared with that in the same sex of the normal control group(P< 0.05). CONCLUSION: The decrease of gelatin-binding activity of Fn is related with burned degree, inbreathed injury and body infection. The increase of the levels of Fn is related with its pathogenesis. The active detect of Fn has different pathophysiologic significance from the content assay.
