摘 要:目的本次严重急性呼吸综合征(SARS)流行中,儿童发病率低、症状轻,推测儿童可能存在保护性抗体。对儿科非SARS患者及医务人员进行血清冠状病毒(变异株)抗体检测,试剂盒为北京华大吉比爱生物技术有限公司冠状病毒(变异株)IgG抗体检测试剂盒(国药试字2003004批号20030501)。方法用酶联免疫吸附法(ELISA),检测2001及2003年的168例非SARS患儿的血清标本,及与这些患儿有密切接触的171份医务人员血清标本,以86份成人非感染非医务人员血清标本做对照。结果非SARS患儿2001年77份血清标本阳性32例(41.56%),2003年91份阳性36例(39.56%),医务人员抗体阳性2例(1.17%),成人对照为0%。儿童冠状病毒(变异株)抗体阳性率有随年龄增长逐渐下降的趋势。抗体滴度中位数在2003年患儿(0.200)、2001年患儿(0.170)、医务人员(0.019)及非感染成人(0.054)间存在极显著性差异。结论在非SARS患儿中冠状病毒(变异株)抗体阳性率高达40%,显著高于成人,与其密切接触的医务人员抗体阳性的检出率与对照组无明显差异,抗体滴度无升高。Objective Children are the high-risk population of respiratory disease, but in this outbreak of sever acute respiratory syndrome (SARS) in China, Children is low morbidity of SARS and mild symptom of children who suffered from SARS. We wondered whether children had protected antibody. The SARS associate coronavirus antibody in serum specimens from childhood patients and medical stuffs were detected. Method ELSIA was used to detect the specific antibody of SARS associate coronavirus in 168 childhood patients and 171 medical stuffs who contacted with these childhood patients closely. The control group was 86 common adults who have no SARS and not medical stuff in Beijing. The antibody titer was expressed as the value of absorb (A). Result The positive cases of SARS coronavirus specific antibody were 32 (41.56%) in 77 children serum that were collected in 2001 year, and that were 36 (39.56%) in 91 children serum from April to May of 2003 year. The positive cases of medical stuffs were 2 (1.17%), meanwhile the common adults was 0%. The antibody positive rate tended to decrease while the child ages went higher. Except diagnosis of pneumonia, there were other diagnosis in positive group, such as: upper respiratory infection, mucocutaneous lymphnode syndrome, exanthema subitum, et al. The median of antibody titer was significant different among childhood patient in 2003 (0.200), in 2001 (0.170), medical stuff (0.019) and no SARS adults (0.054). Conclusion The positive rate of the specific antibody of SARS associate coronavirus in children who were not suffered from SARS was up to 40%, that was significant higher than adult. The medical stuffs who contacted closely with those patients has no significant differences with the common adults in positive rate, the antibody titer was not increased.
