摘 要:目的探讨血清肌酸激酶 (CK)和肌酸激酶同工酶 (CK -MB)作为早期反映芥子气损伤指标的意义。方法以成年家犬为中毒模型 ,中毒剂量为 12mg·kg- 1,皮下注射。随机分为中毒对照组 (4条 ) ,治疗组 (6条 ) ,治疗组动物在中毒后 30min即进行相应的药物治疗。于中毒前所有犬各测一次CK、CK -MB的基础值 ,并于中毒后每日检测心肌酶的动态变化。对中毒致死动物及存活动物 (中毒或治疗后 11d及 2 8d)进行剖验 ,观察心肌细胞在光镜下组织学改变。结果中毒对照组犬于 3d内死亡 3条 ,治疗组犬全部成活。两组犬的CK、CK -MB值在中毒后均大幅度升高 ,中毒对照组尤甚 ,中毒后第 2天治疗组的CK与CK -MB值较中毒对照组分别下降5 3.7%和 4 0 .9% (P <0 .0 5 )。第 4天起治疗组的CK、CK -MB有明显的回落 ,之后趋于正常。 3d内死亡犬心肌细胞损伤较为明显 ,而存活至中毒后 11d或 2 8d犬心肌细胞未见异常。结论血清CK与CK -MB在芥子气中毒早期即显著升高 。Objective To study the significance of changes of CK and CK-MB in early mustard gas poisoning. Methods Ten dogs were used as the animal model, four in control group while six in treatment group. After subcutaneous injection with 12 mg·kg -1 of sulfur mustard (SM), the dogs of the treatment group were treated with medicine. Serum CK and CK-MB were measured before injecting SM and were continually watched for a week after poisoning. Some of the dogs died of poisoning or survived were killed and autopsied. Microscope was used to observe pathological changes in cardiac muscle cells. Results Three dogs in control group died in three days after poisoning, while the dogs in treatment group all survived. CK and CK-MB rose significantly after poisoning, especially in control group. Two days after poisoning, CK and CK-MB were 53.7% and 40.9% less respectively in treatment group than in control group(P<0.05). Four days after poisoning, CK and CK-MB in treatment group began to decline. Cardiomyocytes of died dogs in control group showed obvious damage while no abnormity was found in survived dogs of both group. Conclusion CK and CK-MB whick may increase at early stage of exposure to SM may be sensitive indexes to indicate the damage of SM poisoning.
