双相正压经鼻罩通气治疗慢性阻塞性肺疾病并呼吸衰竭的研究

摘　　要：目的　探讨用双相正压气道通气 (BiPAP)呼吸机经鼻罩辅助通气治疗慢性阻塞性肺病(COPD)合并呼吸衰竭的疗效。方法　 178例老年男性COPD合并呼吸衰竭患者随机分为 2组。试验组 89例 ,在常规药物治疗基础上加用BiPAP呼吸机经鼻罩辅助通气治疗 ,每次 2小时 ,每天 2次 ,使用自主 定时的通气模式 ,持续 10天。对照组 89例常规药物治疗加一般氧疗。结果　试验组患者使用BiPAP呼吸机经鼻罩通气治疗后心率、呼吸频率下降 ,血氧饱和度上升 ,PaCO2 降低 ,PaO2 增加 ,肺功能及缺氧状况改善 ,与对照组比较 ,差异有极显著意义 (P均 <0 .0 1)。结论　用BiPAP呼吸机经鼻罩辅助通气治疗老年男性COPD合并呼吸衰竭是一种无创有效的措施 ,能在短期内明显改善临床症状。Objective To investigate the therapeutic effec t of assisted ventilation of bi-level positive airway pressure respirator via n ose mask on chronic obstructive pulmonary disease(COPD) combined with respirator y failure.Methods 178 aged male patients with COPD combined wit h respiratory failure were randomly divided into two groups.89 patients in trial group,on the basis of routine drug treatment, were treated by assisted ventilat ion of bi-level positive airway pressure respirator via nose mask,two hours onc e, twice a day,by the auto/timing ventilation way. 89 patients in control group were treated by routine drug therapy combined with oxygen treatment.Resu lts After treatment by respirator ,the heart rate, PaCO 2 and respira tory frequency of patients in trial group were decreased, however,the degree of blood oxygen saturation and PaO 2 were increased .The lung function and anoxic state of patients in trial group were significantly improved,as compared with th ose in control group (P<0.01).Conclusion The assisted vent ilation therapy of bi-level positive airway pressure respirator via nose mask f or aged male patients with COPD combined with respiratory failure is an effectiv e and noninvasive method,which can improve patient's clinical symptoms in a shor t time,promote patient's rehabilitation and enhance patient's quality-of-life.
