摘  要:目的 探讨异氟醚预处理对大鼠全脑缺血再灌注损伤保护作用的机制。方法 以雄性Wistar大鼠 (2 5 0~30 0g)为研究对象 ,采用 4动脉阻断法制作全脑缺血模型。根据模型判断标准 ,筛选大鼠 78只。随机分为 4组 :假手术组 (SH)、缺血再灌注组 (ISC)、缺血预处理组 (IPC)、异氟醚预处理组 (ISOPC)。每组按缺血后再灌注时间分为 3个亚组 :再灌注 6h、再灌注 2 4h、再灌注 72h。对脑组织标本行HE染色及免疫组织化学法检测Bcl 2蛋白表达。结果 CA1区Bcl 2蛋白表达均在再灌注 72h之后。假手术组几乎没有表达。ISC组表现为只有在部分存活的锥体细胞内表达。在IPC组和ISOPC组CA1区锥体细胞呈强阳性表达 ;且在血管内皮细胞内亦有表达。结论 异氟醚预处理对大鼠全脑缺血再灌注损伤保护作用的机制可能为诱导抗凋亡基因Bcl 2蛋白表达 ,通过抑制缺血诱导的神经细胞凋亡而产生保护作用 ,具有拟IPC作用。Objective To investigate mechanism of isoflurane preconditioning's neuroprotection against global cerebral ischemia in rat.Methods Global cerebral ischemia was induced by occlusions of bilateral common carotid and vertebral arteries (four vessels).Seventy-eight male healthy Wistar rats (weighing 250~300g) were selected according to model criteria and randomly divided into four groups:sham operation group (SH),ischemia group (ISC),ischemia preconditioning group (IPC),and isoflurane preconditioning(ISOPC).Each group was divided into three subgroups according to reperfusion time:6h,24h and 72h.Every cerebrum was resected and stained using hematoxylin-eosin (HE) and immunohistochemical method to evaluate the expression of Bcl-2 protein.Results Bcl-2 protein expressed in CA1 after 72h reperfusion in ISC,IPC and ISOPC group.There was almost no expression of Bcl-2 protein in SH group.Bcl-2 protein expressed in part of survival pyramidal neuron cell in ISC.Whereas in IPC and ISOPC group,Bcl-2 protein expression immunoreactivity significantly increased;and also expressed in endothelial cell of vessel.Conclusion The expression of Bcl-2 protein in the CA1 area following preconditioning is closely related to the acquisition of suppressing the apoptosis of neural cells.Isoflurane preconditioning can mimic the role of IPC to suppress the apoptosis of neural cells following ischemia and induce neuroprotection.
