摘  要:目的了解死亡患者医院感染的基本情况,探讨死亡原因与感染的关系。方法对我院2002年度死亡病历共230份,采取回顾性的调查分析。结果230例死亡病历中有医院感染72例125例次,医院感染率为313%,是同期医院感染的974倍;4739%的死亡与感染有关;72例有医院感染的死亡病例平均住院564d,比全院平均住院日(173d)多391d;死亡病历中≥两种基础疾病204例;各种恶性肿瘤103例,占死亡总数的4478%;呼吸系统感染占医院感染的首位;在送检的各类标本中检出G-菌37株,G+菌25株,真菌22株。结论医院感染是加速和导致患者死亡的重要原因;各种恶性肿瘤患者是医院感染的主要人群;呼吸系统感染是预防的重点;G-菌为医院感染的主要致病菌。OBJECTIVE To explore the basic nosocomial infection data of the dead cases and discuss the relationship between death reasons and infection. METHODS Two hundred and thirty dead cases were investigated and analyzed in 2002. RESULTS In 230 dead cases, there were 72 cases with 125 case times of nosocomial infection. The rate of their nosocomial infection was 31 3%, which was 9 74 times higher than that at the same period of the hospital; 47 29% of dead cases were related to nosocomial infection; the average hospitalization time of these 72 dead cases was 56 4 days, which was 39 1 days above the whole average hospitalization time (17 3 days). Of all dead cases, 204 had two or more basic diseases; 103 (44 78%) had various malignant tumors, respectively. Respiratory system infection was the primary nosocomial infection. All detected samples included 37 Gram negative bacteria, 25 Gram positive bacteria, and 22 fungi strains. CONCLUSIONS Nosocomial infection is an important reason that accelerated and led to patients death. Critical patients and all kinds of malignant tumors patients are the high risk groups of nosocomial infection; respiration system infection is the focal point in prevention; Gram negative bacteria were the main nosogenesis of nosocomial infection.
