老年人生长激素-胰岛素样生长因子-1轴活性与血糖、血脂的关系

摘　　要：目的:探讨老年人生长激素-胰岛素样生长因子-1(insulin-likegrowthfactor-1,IGF-1)轴的改变对糖代谢、脂代谢的影响。方法:2001-12/2003-06哈尔滨市第一医院门诊年龄≥60岁的老年人372例。符合纳入标准老年人188例,年龄60～82岁。依据血糖、血脂值分为3个亚组为血糖升高组(n=31)、血脂异常组(n=123)及健康老年组(n=34)。选择184例哈尔滨道里区健康自愿者为青壮年对照组(n=184)。测定188例老年人(≥60岁)和正常青壮年人(年龄30～40岁)的血清基础生长激素及IGF-1水平。同时测定空腹及餐后2h血糖、血脂。结果:老年人的基础生长激素值犤(0.783±0.079)μg/L犦明显低于正常青壮年对照组犤(1.706±0.210)μg/L犦(t=4.51,P<0.001),IGF-1值犤(146.18±39.83)μg/L犦也显著低于正常青壮年对照组(196.03±61.81)μg/L,t=2.91,P<0.01)。血糖升高组、血脂异常组老年血清IGF-1值显著低于健康老年组(t=5.96～8.31,P<0.01),老年血糖升高组与健康老年组的生长激素值比较,差异无显著性意义(P>0.05),而血脂异常组老年人血清生长激素值显著低于健康老年组(t=6.35～7.23,P<0.01)。老年人血清基础生长激素,IGF-1的水平与三酰甘油、总胆固醇、低密度脂蛋白胆固醇呈显著负相关(r=-0.37～-0.61,P<0.01);AIM:To investigate the effect of changes grow hormone(GH) insulin like growth factor 1(IGF 1) axis activity on the glucose and lipid metabolisms in elderly people.<METHODS:A total of 188 cases,aged 60 to 82 years old,were selected from the 372 elderly patients,aged no younger than 60 years old in the Outpatient Department of the First Hospital of Harbin City from December 2001 to June 2003.The 188 cases were subdivided into increased plasma glucose group(n=31),abnormal plasma lipid group(n=123) and elderly healthy group(n=34).Other 184 healthy volunteers in the Daoli District of Harbin City were taken as the young group.Serum basic GH and IGF 1 levels,fasting plasma glucose(FBG),2 hour postprandial plasma glucose(PBG),fasting plasma lipid(FPL) and 2 hour postprandial plasma lipid(PBL) were determined in all the groups.<RESULTS: The serum levels of GH and IGF 1 in the elderly group[(0.783± 0.079),(146.18± 39.83) μ g /L] were all significantly lower than those in the young group[(1.706± 0.210), (196.03± 61.81) μ g/L](t=4.51,P< 0.001;t=2.91,P< 0.01).The basic serum levers of IGF 1 in the increased plasma glucose group and abnormal plasma lipid group were significantly lower than that in the elderly healthy group(t=5.96 to 8.31,P< 0.01),and there was no significant difference between the increased plasma glucose group and elderly healthy group(P >0.05),while it was significantly lower in the abnormal plasma lipid group than in the elderly healthy group(t=6.35 to 7.23,P< 0.01).The serum levels of GH and IGF 1 in the elderly people had significantly negative correlation with triacylglycerol(TG),total cholesterol(TC) and low density lipoprotein cholesterol(LDL C)(r=- 0.37 to - 0.61,P< 0.01),but had positive correlation with high density lipoprotein cholesterol(HDL C)(r=0.32,P< 0.05;r=0.35,P2< 0.01).The serum IGF 1 level was also negatively correlated with FBG (r=- 0.31,P< 0.02).<CONCLUSION:The GH/IGF 1 axis activity is decreased in the elderly people.The GH/IGF 1 axis as an independen
