平肝降压胶囊对原发性高血压患者血浆内皮素和血管紧张素Ⅱ的影响

摘　　要：目的:观察平肝降压胶囊对原发性高血压患者治疗过程中血压变化与血浆内皮素和血管紧张素Ⅱ的关系,以探讨其发生疗效的作用机制。方法:2002-11/2004-05西安交通大学第二医院急诊科及中西医结合科住院和门诊原发性高血压患者160例,以随机双盲法分设平肝降压胶囊治疗组、清脑降压片对照组各80例,治疗12周。观察患者治疗前后临床症状、血压、血液流变学、血浆内皮素及血管紧张素Ⅱ(AngⅡ)的变化。结果:平肝降压胶囊具有明显的降低血压和改善临床症状,降低血液黏稠度和血浆内皮素(72.22±12.37vs59.08±12.77,ng/L)及血管紧张素Ⅱ(84.75±7.34vs61.37±8.31,ng/L)水平作用(P<0.05或P<0.01)。结论:平肝降压胶囊可能通过降低血浆内皮素、AngⅡ的水平,改善血黏度,从而达到降低血压的作用。AIM: To observe the relationship among the change of blood pressure, plasma endothelins and angiotensin Ⅱ (Ang Ⅱ ) in patients with primary hypertension during the treatment with pinggan jiangya capsule, so as to investigate the therapeutic effect and the mechanism.<METHODS: A total of 160 patients with primary hypertension, selected from the Department of Emergency, Integrated Traditional Chinese Medicine and Western Medicine, and out patient clinic, the Second Hospital, Xi'an Jiaotong University from November 2002 to May 2004, were randomly and double blindly divided into treatment group and control group treated by pinggan jiangya capsule and qingnao jiangya pian, respectively, with 80 patients in each group. All patients were treated for 12 weeks in order to observe the changes of clinical symptoms, blood pressure, hemorheology, plasma endothelins and Ang Ⅱ before and after treatment.<RESULTS: Pinggan jiangya capsule could decrease the blood pressure and improve the clinical symptoms, and also decrease the blood hypercoagulability and the level of plasma endothelins [(72.22± 12.37) vs (59.08± 12.77)ng/L]and AngⅡ [(84.75± 7.34) vs (61.37± 8.31)ng/L] obviously ( P< 0.05 or P< 0.01).<CONCLUTION: Pinggan jiangya capsule can improve the blood hypercoagulability according to the decreasing of the level of plasma endothelins and AngⅡ , so as to decrease the blood pressure.
