摘  要:背景:针刺内关可以抑制心肌缺血再灌注损伤的进程,对心肌组织肌酸激酶、丙二醛等有明显的调节作用。目的:探讨针刺内关抗心肌缺血再灌注损伤作用的内在机制。设计:随机对照的实验研究。地点、材料和干预:本实验在湖南中医学院针灸经络研究所实验室完成。实验选用大耳白兔,雌雄不限,随机分为4组:空白组、模型组、针刺内关组、针刺列缺组,每组8只。G6805电针仪电针家兔内关穴、列缺穴各20min。主要观察指标:白兔心肌组织一氧化氮、一氧化氮合酶含量变化。结果:心肌缺血再灌注后模型组一氧化氮及一氧化氮合酶含量明显高于空白组(P<0.01);针刺内关穴组白兔心肌组织一氧化氮及一氧化氮合酶含量犤(0.69±0.15)mmol/g,(0.800±0.150)μkat/g犦明显低于模型组犤(1.67±0.27)mmol/g,(2.434±0.267)μkat/g犦(P<0.01)。针刺列缺组白兔心肌组织一氧化氮和一氧化氮合酶含量与模型组比较,差异无显著性意义(P>0.05)结论:电针内关穴对心肌缺血再灌注损伤心肌的保护作用可能与抑制一氧化氮及一氧化氮合酶的合成、释放,从而减轻其对心肌细胞的损害有关。BACKGROUND:Acupuncture at the neiguan acupoint(PC 6) is able to inhibit the development of injury of ischemia reperfusion of myocardium and to regulate the creatine kinase(CK) and malonyldialdehyde(MDA) obviously.<OBJECTIVE:To study the mechanism of electroacupuncture at neiguan acupoint in the treatment of myocardial ischemia reperfusion injury.<DESIGN:A randomized controlled experimental study.<SETTING,MATERIALS and INTERVENTIONS:The experiment was completed in the laboratory of Acupuncture Moxibustion and Meridians,Hunan College of Traditional Chinese Medicine. The big ear rabbits, male and female, were randomized into 4 groups with 8 rabbits in each group: blank group, model group, neiguan group, and lieque(LU 7) group.The electric stimulator G6805 was used to give stimulation at the neiguan and lieque acupoints for 20 minutes respectively. <MAIN OUTCOME MEASURES:Changes of contents of nitric oxide(NO) and nitric oxide synthase(NOS) in the myocardial tissue of the rabbits.<RESULTS:The NO and NOS contents after myocardial ischemia reperfusion in the model group were obviously higher than those in the blank group(P< 0.01).The NO and NOS contents were significantly lower in the neiguan group[(0.69± 0.15) mmol/g,(0.800± 0.150) μ kat/g] than those in the model group[(1.67± 0.27) mmol/g,(2.434± 0.267) μ kat/g](P< 0.01),while there were no significant differences between the lieque group and model group(P >0.05).<CONCLUSION:The protection of electroacupuncture at the neiguan acupoint on myocardium after ischemia reperfusion injury is possibly related to the inhibition of synthesis and release of NO and NOS,which can relieve its damage to cardiomyocytes.
