摘　　要：目的探讨来氟米特对肾大部切除大鼠肾脏纤维化的影响。方法将大鼠随机分为假手术组和手术组,术后3d再将手术组大鼠随机分为来氟米特组和Nx组。8周后测尿蛋白 ,血肌酐和肝功能 ;取肾组织做病理检查 ;用免疫组织化学法检查巨噬细胞特异性抗原CD68的表达。结果Nx组大鼠肾脏出现肾小球硬化和肾间质纤维化 ;和Sham组相比CD68表达明显升高 (P<0.05);LEF组肾脏纤维化明显减轻 ,CD68表达比Nx组明显下降 (P<0.05)。结论来氟米特可抑制巨噬细胞增殖和浸润 ,减轻肾脏纤维化 ,从而保护肾功能。Objective To study the effects of leflunomide on renal fibrosis with5/6renal ablation in rats.Methods The rats were randomly divided into2groups:Sham group(n=10)and5/6nephrecˉtomy group(n=30).Nine rats in the group that had5/6nephrectomy died during surgery or in the following48hours.In the third day after surgery5/6nephrectomy rats were randomly subdivided in two groups(LEF5mg.kg-1.day-1by daily gastric gavage,n=11and Nx group,n=10).Nx and Sham rats were received equal volume water by daily gastric gavage.All rats were allowed to have food and water freely.In eight weeks after the treatment with leflunomide,the following studies were performed in every group.All of the items such as body weight,the amount of24hours urine protein,serum creatinine(Scr),liver enzyme were measured.The renal tissues of three group were observed with light microscope after periodic acid-Schiff(PAS)and Masson staining.Immunohistochemical staining was used to estimate the expression of CD68in renal cortex and the results were analyzed seˉmi-quantitatively with the pathological image analysis system.The results were analyzed semi-quanˉtitatively with the pathological image analysis system.Results Leflunomide treatment prevented the progressive increment in serum creatinine and the proteinuria observed in the5/6nephrectomized rats during the eight weeks of observation(P<0.05).The degree of weight gain was comparable in the LEF-treated and sham-operated rats,whereas weight gain was less in untreated5/6nephrectomized rats than above two groups(P<0.05).Expression of CD68-positive cells was increasingly strong in the glomeruli,tubules,and interstitium of remnant kidneys in Nx rats during eight weeks.Their expression was reduced significantly with leflunomide treatment(P<0.05).The histologic observati_ ons were shown severe mesangial expansion,extensive capillary loops ollusion,tubular atrophy and interstitial fibrosis in eight weeks in Nx group,severe mesangial expansion,extensive capillary loops ollusion,glomerulosclerosis,tubular atr
