丁型肝炎患者血清HDV_IgM检测结果及资料分析

摘　　要：目的　分析丁型肝炎的年龄分布、男女比例与乙型肝炎病毒 (HBV)重叠感染以及肝功能指标变化的情况。方法　用酶联免疫吸附法 (ELISA)对患者血清进行HBV抗体IgM(抗_HDV_IgM)的检测 ,同时观察肝功能指标的变化。结果　两组人群检测丁型肝炎抗体IgM(1)乙型肝炎人群组 ,丁型肝炎病毒 (HDV)感染率 4 9% (137/2 789) ,(2 )非乙型肝炎人群组 ,HDV感染率 0 13% (1/ 795 )。 138例丁型肝炎患者 ,肝功能指标异常率 81 2 % (112 /138) ,乙型肝炎患者未合并HDV感染 ,肝功能指标异常率 3 2 % (85 / 2 6 5 2 ) ,两组比较P <0 0 5 ,有显著性差异。HDV感染者以中老年多见 ,占 72 5 % ;且男多于女 ,男女比例为 2 9∶1。结论　乙、丁二型肝炎病毒重叠感染时 ,肝功能指标大多异常 ,临床上引起严重的和进行性肝病 ,并与暴发型肝炎和肝硬化密切相关 ,提示中老年乙型肝炎患者是丁型肝炎易感者 ,应积极治疗乙型肝炎 ,并做好自我保护 ,避免再染丁型肝炎。Objective To analysis HDV infection age range,male/female proportion and coinfection with HBV as well as the index changes of liver function. Methods Anti-HDV-lgM test and the index changes of liver function were determined by ELISA. Results Two Groups of people were tested Anti-HDV-IgM.(1)In HBV group,the rate of HDV infection was up to 4.9%(137/2 789);(2) While in non-HBV group was 0.13%(1/795).There were 138 HDV patients.The abnormal rate of their liver function index was 81.2%(l12/138)while the HBV patients who weren’t co-infected with HDV,took the rate of 3.2%(85/2 652).Comparing with two groups (P<0.05),there was marked difference.The middle-aged and old people were more easily infected by HDV,taking the rate of 72.5% and the male people were more than the female,taking the rate of 2.9 to 1.Conclusion When HBV and HDV were co-infected,the index of liver function was usually abnormal that in clinic would lead to serious liver disease and have close connection with breaking-out hepatitis and cirrhosis.Advice the middle-aged and old HBV patients are easily infected by HDV,who should positively have HBV treatment and make good self-protection to avoid being infected by HDV.
