摘 要:目的研究多层螺旋CT肺容积指标与肺功能(PFT)的相关性及其在阻塞性肺通气功能障碍中的特点。方法正常对照组38例,阻塞性肺通气功能障碍24例,其中20例诊断慢性阻塞性肺疾病(COPD),均在1周内完成PFT及CT检查。采用多层螺旋CT(MSCT)于最大吸气末和最大呼气末分别扫描全肺,用Pulmo肺定量软件测定全肺容积。结果MSCT的肺容积指标其中的最大吸气末容积(Vin)与PFT的肺总量的相关性最佳(r=0.961,P<0.01);最大呼气末容积(Vex)与残气量(RV)、容积差(Vin-Vex)与最大肺活量(MVC)、容积比(Vex/Vin)与残气量与肺总量比(RV/TLC)也存在着较好的相关性(r=0.899,P<0.01;r=0.772,P<0.01;r=0.614,P<0.01)。阻塞性肺通气功能障碍者Vin、Vex、Vex/Vin均有显著升高(P<0.058),与PFT的表现相一致。结论MSCT测定的肺容积指标与PFT有很好的相关性,可以用于评估COPD患者的肺功能状况。Objective To study the correlation between MSCT in measuring lung volumes and pulmonary function test (PFT),and CT features of obstructive pulmonary abnormalities. Methods In 62 cases,there were 38 healthy controls,24 patients with obstructive pulmonary abnormality, in which 20 with chronic obstructive pulmonary disease (COPD).CT scan and PFT were performed within one week in all cases. The total lung was respectively scanned at the end of full inspiration and full expiration with MSCT. The total lung volumes were measured by Pulmo pulmonary quantitative software. Results The full inspiration volume(Vin) best correlated with total lung capacity(TLC)(r = 0.961, < 0.01),and the full P expiration volume(Vex) well correlated with residual volume(RV) (r = 0.899, < 0.01), as well as Vin-Vex with maximum vital capacity P (r = 0.772,P < 0.01),Vex/vin with RV/TLC (r = 0.614,P < 0.01).Vin, Vex and Vex/Vin significantly elevated in patients with obstructive pulmonary abnormalities compared with those in controls (P < 0.058). Conclusion Significant correlated with PFT in measuring lung volume, MSCT can be used to assess the pulmonary function of patients with COPD. [
