摘 要:目的:探讨糖耐量低减(IGT)患者的红细胞胰岛素酶活性(EIA)与胰岛素抵抗(IR)的关系。方法:用放射酶分析法检测50例IGT患者和20例正常对照的EIA及相关指标,并计算稳态模型胰岛素抵抗指数(HOMA-IR)以评价机体的胰岛素敏感性。结果:IGT患者的EIA、空腹血清胰岛素(FINS)和HOMA-IR指数显著高于正常对照(P<0.01),伴空腹血糖异常的IGT患者与空腹血糖正常的IGT患者相比,前者的EIA、FINS、HOMA-IR显著高于后者(P<0.01或0.05)。直线回归分析表明,IGT患者的EIA与FINS、HbA1c和HOMA-IR指数呈显著正相关(P<0.01或0.05)。结论:IGT患者的红细胞胰岛素降解速度显著高于正常,并与其IR的发生、发展有关。Objective To investigate the relationship between insulinase activity of erythrocytes (EIA) and insulin resistance in patients with impaired glucose tolerance (IGT). Methods EIA was measured with the method of radio assay of enzyme activity in 50 patients with IGT and 25 healthy subjects. Fasting plasma insulin (FINS) and other related indices were also measured in all the subjects. Moreover,homeostasis model analysis (HOMA) insulin resistance index (HOMA IR) was calculated for estimation of insulin sensitivity. Results In patients with IGT,EIA,FINS and HOMA IR index were increased as compared with healthy subjects. Moreover,EIA,FINS and HOMA IR in IGT patients accompanied with impaired fasting glucose (IFG) were higher than those in IGT patients with normal fasting plasma glucose(P< 0.01). Correlation analysis showed that EIA were positively correlated with FINS,HbA1c and HOMA IR,respectively,in all patients with IGT (r=0.56,0.54,0.47,respectively,P< 0.01) and controls (r=0.51,0.44,0.49,respectively,P< 0.01). Conclusions The rate of insulin degradation in erythrocytes is accelerated in patients with IGT,and increased insulin degradation may result in their insulin resistance.
