葶苈小复方治疗肺心病肺动脉高压临床研究  

Clinical Observation on the Effect of Tinglixiaofufang(葶苈小复方) in Curing Chronic Cor Pulmonale and Pulmonary Atrery Hypertention

摘　　要：目的观察葶苈小复方对肺心病肺动脉高压患者动脉血气、肺阻抗血流图和血液流变学的影响。方法将39例患者随机分为两组,治疗组采用葶苈小复方治疗,对照组采用络活喜治疗;观察两组临床疗效,并比较两组治疗前后动脉血气、肺阻抗血流图和血液流变学的变化。结果两组临床疗效相似;治疗组可明显降低PaCO2,明显升高PaCO2、O2AST,延长B-Y间期,增加HS,减缓HR,降低mPAP,与对照组比较无显著差异;治疗组全血粘度(低切、高切)、血浆粘度、红细胞比积均较治疗前降低,差异有显著性,而对照组各指标治疗前后比较差异无显著性。结论中药葶苈小复方通过泻肺活血以降低血液粘度,从而降低肺动脉高压,并在血液流变学改善方面较西药有着明显的优势。Objective:To observe the effect of Tinglixiaofufang(葶苈小复方) on blood gas indices,pulmonary impedance rheogram and hemorrhedogy of patients with chronic cor pulmonale and pulmonary artery hypertention.Methods:39 patients were randomly divided into two groups.Patients the treatment group were treated with Tinglixiaofufang,and those in the controlled group were treated with Amldipine.To observe the effect and detect the blood gas indices,pulmonary impedance rheogram and hemorrhedogy of both groups′s before and after treatment.Results:The curative efficacy of the two groups were similar.And both therapies of the groups' could decrease PaCO2 and improve PaO2,O2AST remarkably.It also can extend B-Y interval,increase HS,reduce HR,reduce mPAP.The treatment group's levels of blood viscosity (low shear rate,high shear rate),plasma ratio viscosity and hematocrit became lower compared with before treatment.The difference was remarkable,while the indices in the controlled group didn't manifest obvious difference on those indices compared with before treatment.Conclusion:Thingixiaofufang,the Chinese herb can reduce pulmonary artery hypertention through activating blood by purging lung and decrease blood viscosity,and has more superiority on improving hemorrhedogy than west medicine.
