摘  要:目的 探讨神经生长因子 (nervegrowthfactor ,NGF)和睫状神经营养因子 (ciliaryneurotrophicfactor ,CNTF)对大鼠同一坐骨神经再生纤维超微结构的影响。方法 用自行研制的梭形双通道桥接管桥接 3 6只大鼠坐骨神经 10mm缺损 ,根据梭形管的两支内加入的药物不同将动物随机分为A组 (两支管内均加入几丁糖 )和B组 (两支管内加入几丁糖后 ,再分别加入NGF和CNTF)。于术后 4、8、16周取再生神经的中段行透射电镜观察 ,并对 8、16周再生神经的有髓和无髓纤维的面积、有髓纤维轴突直径、髓鞘厚度行计量分析。结果 A组两支管内再生神经纤维的种类、数量无显著差异。B组NGF侧再生纤维以有髓纤维为主 ,且其髓鞘较厚 ,轴突直径较大 ,而CNTF侧再生神经有髓和无髓纤维均增加 ,无髓纤维面积较NGF侧大 ,两者差异显著 (P <0 0 5 )。结论 NGF对有髓纤维的再生和髓鞘形成的作用较强 ,CNTF可同时促进有髓和无髓纤维再生 ,但其促进有髓纤维再生的作用不及NGF。Objective To discuss the effects of nerve growth factor (NGF) or ciliary neurotrophic factor (CNTF) on the ultrastructures of regenerative nerve fibles. Methods Thirty six Sprague-Dawley rats with 10 mm gap of sciatic nerve were randomly divided into two groups which had been bridged with the new double channel nerve conduit of fusiform shape (groups A and B, n=18 in each group). In group A, 200 μl chitin for medical use was injected into the conduit. In group B, the two branches of the conduit respectively contained 100 μl chitin and 5 μl NGF or CNTF. At 4, 8, and 16 weeks after operation, we evaluated the ultrastructures of regenerative nerve and analyzed the difference by counting method. Results There was not significant difference in the regenerative nerve fibres between the two channels in group A, but in group B, the number of myelinated fibre of regenerative nerves of NGF lateral branch was much more, and the myelin sheath was also thicker. Both the myelinated fibre and unmyelinated fibres in the CNTF lateral branch increased. The diameter of axon and the area of myelinated fibre of NGF lateral branch were larger than the ones of CNTF labteral branch, and there was significant difference between them, but its area of unmyelinated fibre was smaller than that of CNTF labteral branch. Conclusion NGF can significantly promote regeneration and myelinization of myelinated fibres. CNTF can promote the regeneration of both myelinated and unmyelinated fibres, but the ability of regeneration and myelinization of myelinated fibre is inferior to NGF.
