胃癌手术病人住院费用影响因素调查分析

摘　　要：目的 :探讨外科病人住院床日与费用的影响因素 ,提出缩短住院床日和降低医疗费用的有效途径。方法 :抽取住院胃癌手术病例 10 2例 ,调查其住院期间医疗质量与费用的有关因素 ,共 65项 ,采用一元与多元分析 ,研讨各种因素与住院床日、住院费用之间的关系。结果 :影响住院床日的主要因素是 :检验、伴随病、确定手术时间、院内感染、拆线时间、术后住院时间、年龄等 ;影响住院费用的主要因素是 :职业、住院天数、诊断水平、手术情况、术后情况、辅检、医疗保健制度等。结论 :加强医院管理 ,提高诊疗质量与服务水平 ,可进一步缩短住院床日 ,降低住院费用 ,减轻病人经济负担。Objective To explore factors influencing duration of hospitalization and medical expenses in surgical patients, and the way to reduce the duration of hospitalization and medical expenses effect methods.Methods One hundred and two patients undergoing gastric carcinoma operation were selected to investigate 65 factors factors such as medical quality and medical expenses with duration of hospitalization by monoanalysis and multianalysis.Results The main factors that affect the duration of hospitalization were examinations performed associated diseases, the time confirmed when to receive operation, nosocomial infection, times of removing sutures, post operative hospital stay, age and so on. The main factors that affect medical expenses were occupation, diagnosis level, operation, postoperation, laboratory test, medical care system and so on. Conclusions Improving hospital administration, medical quality and service level can shorten the duration of hospitalization and reduce medical expenses,thus alleviate patients' costs.
