摘 要:目的为了较全面地评价溶栓治疗效果 ,本实验设计了一个既能观察血栓溶解过程 ,同时又能观察组织微循环状况的溶栓治疗动物模型。方法以大鼠提睾肌动脉作为靶血管 ,静脉注射血卟啉(1mg/kg)后 ,以紫外光 (波长455nm)照射靶血管一定部位 ,通过光化学法制造血栓 ,血栓形成后10min输葡激酶 (1mg/kg)静脉溶栓 ,以生理盐水作为对照。显微镜下直接观察血栓形成与溶解过程以及微循环血流状态 ,用显微摄像系统记录分析图像 ,并通过激光多普勒血流量图像仪测组织的血流量 ,从而评价血栓的溶解效果及溶栓前后组织的微循环状态与血流灌注情况。结果两组的再通率及再通程度无统计学差别 (P>0.05) ,而溶栓组的血栓溶解时间较对照组减低 ,统计学差别显著(P<0.05) ;血栓溶解后 ,组织血流量和微血流流速的恢复值均较正常时减少 ,统计学差别显著(P<0.05)。结论葡激酶具有一定的促血栓溶解效果 ,主干血管血栓溶解后 ,该血管供应的下游组织在微循环灌注等方面并不能完全恢复至正常状态。通过此模型 ,既可以评价溶栓剂溶解血栓的效能 ,又可衡量溶栓后组织的再灌注情况。Objective To develop an experimental animal thrombolysis model to evaluate not only the reˉcanalization of the thrombosed large vessels,but also the microcirculatory consequence of downstream tisˉsue.Methods23male SD rats were randomly assigned to thrombolysis group(n=14)and control group(n=9).In all animals,thrombosis was induced photochemically in cremaster arteries by injecting hematoporphyrin(1mg/kg)and then exposing the target arteries to ultraviolet light(455μm).After10minutes of occlusion,the thromboˉsis group received recombinant staphylokinase(1mg/kg.1/3bolus injection followed by2/3infusion over30minˉutes),and the control group was administrated the same volume of saline.The process of the clot lysis in large vesˉsels and the circulatory conditions in microvasculars were recorded through vital microscope equipped with video recording system,the lytic and microcirculatory parameters,such as degree of recanalization(DR),microvessel flowvelocity(MFV),time to lysis(TL)and formation of microemboli,were measured by computerised image analˉysis,and the terminal tissue blood flow(TBF)were assayed with Laser Doppler Perfusion Imager.Results Beˉtween the two groups,there was no significant difference of lysis rate and DR(P>0.05),but TL decreased in thrombolysis group and reached statistical significance(P<0.05).In both groups,MFV and TBF after thrombolytˉic therapy didn't recover completely compared with those before thrombosis,and these differences were statistically significant(P<0.05).Conclusions thrombolytic therapy with staphylokinase can enhance the lysis of thrombus,but can't recover the downstream flowcompletely.With this model,both recanalization of thrombosed large vessels and the microcirculatory consequence of downstreamtissue can be evaluated simultaneously.
