摘　　要：目的　通过对罕见大脑后动脉P2 段巨大动脉瘤治疗及相关文献的复习 ,探讨P2 段巨大动脉瘤的诊断及治疗方法。方法　 38岁女性患者因突发头痛 ,经MRI、CT扫描及DSA诊断为右侧大脑后动脉P2 段巨大动脉瘤 (3.5cm× 4 .0cm )。结果　全麻下经颞下入路近端夹闭P2 段载瘤动脉后成功切除动脉瘤 ,术后患者恢复良好。结论　P2 段巨大动脉瘤少见 ,手术阻断近端载瘤动脉或夹闭动脉瘤颈并切除为其主要治疗方法。Objective To analyze and discuss the diagnosis and s urgical treatment on rare giant aneurysm of the P 2 segment of posterior c erebral artery (PCA), by reviewing the treatment and literatures.Methods A 3.5 cm×4.0 cm mass lesion in the right mediobasal temporal regi on was detected by the MRI and CT scan in a 38 years old female patient who was admitted for sudden headache, and diagnosed as a giant P 2 aneurysm on DSA . Results The P 2 segment was clipped proximal to the a neurysm, which was totally excised with subtemporal approach under endotrach eal general anaesthesia and the patient got a good recovery.Conclusion Trapping or neck clipping and resection of aneurysm is the main method f or treatment of the rare P 2 giant aneurysm.
