房性早搏始动的阵发性心房颤动的动态心电图分析  被引量:24

摘  要:了解房性早搏 (简称早搏 )与阵发性心房颤动 (简称房颤 )的关系。通过 2 4h动态心电图分析 2 4例阵发性房颤患者 ,观察其 2 4h房颤的发作阵数 ,房早的发作情况 ,诱发和未诱发房颤的房早的联律间期、房早前周长、房早前12 0s房早频率等。结果 :诱发房颤的房早比未诱发房颤的房早显著提前 (35 5 .9± 32 .8msvs 4 0 8.4± 4 1.7ms,P <0 .0 1) ,房早是否诱发房颤还与房早前周长长 (85 4± 10 6msvs 74 2± 113ms ,P <0 .0 1)及房早前 12 0s的房早频率快 (2 1.7± 15 .3次 /分vs9.79± 7.92次 /分 ,P <0 .0 1)有关。结论 :房早是否始动阵发性房颤不仅与房早的联律间期有关 ,而且与房早前周长及其前 12 0s房早的频率有关。To investigate the relationship between premature atrial contraction (PAC) and paroxysmal atrial fibrillation (AF), the holter of 24 patients with paroxysmal AF were analyzed .The onset numbers of AF , the frequency of PAC ,the onset number of atrial tachycardia(AT), the coupling interval of PAC ,the circle length before PAC, the frequency of PAC in 120 seconds before the AF were observed . Results: The coupling intervals of PACs who trigger AF were be shorter than that of those who fail to trigger AF(355.9±32.8 ms vs 408.4±41.7 ms, P <0.01). The numbers of PACs in 120 seconds before the onset of the atrial fibrillation seem more than those in 120 seconds before common ones(21.7±15.3 bpm vs 9.79±7.92 bpm, P <0.01)and whether PAC triggering AF or not was closely correlated with the circle length before the PACs (854±106 ms vs 742±113 ms, P <0.01). Conclusion: Whether PACs trigger AF or not are tightly associated with PACs coupling intervals , the circle length before PAC and the frequency of PACs in 120 seconds before the onset of the atrial fibrillation .
