摘  要:目的 验证人皮肤真皮层在增生性瘢痕 (hypertrophicscar ,HS)形成中的决定作用 ,探讨HS的发病机制。方法 在裸鼠背部体表分别移植人的全厚皮片、刃厚皮片和中厚皮片 ,以及大鼠的全厚皮片 ,观察皮片干痂脱落后局部瘢痕增生的大体情况及组织学变化。结果 只有移植人的全厚皮片才会发生明显的瘢痕增生 ,而移植人的刃厚皮片以及大鼠的全厚皮片则根本不发生瘢痕增生 ,移植人中厚皮片虽然部分可以发生一定程度的瘢痕增生 ,但增生程度及发生率均明显不如移植人的全厚皮片。结论 人皮肤内在的因素是HS发生的决定因素 ,这种决定因素存在于人皮肤的真皮层。Objective To clarify the roles of human dermis in the development of human hypertrophic scar (HHS) formation and to explore the mechanisms of HHS. Methods Full-thickness skin graft, epidermal skin graft, intermediate split thickness skin graft of human, and full-thickness skin graft of rats were transplanted onto nude mice. Scars that developed after skin graft rejection were studied macrosopically and histologically. Results Obvious hypertrophic scars developed only in the human full-thickness skin graft group. Though scar proliferation could also be seen in the human intermediate split thickness skin graft group, the proliferation was much less than that in the full-thickness skin graft group. No scar proliferation was found either in the human epidermal skin graft group or rat full-thickness skin graft group. Conclusion Some internal factors in the human skin are the determining factors for the development of human hypertrophic scar, and these factors lie in the human dermis.
