慢性病态细胞综合征临床特点分析

摘　　要：目的　初步探讨综合医院内科住院患者慢性病态细胞综合征 (chronicsickcellsyndrome ,CSCS)临床特点。方法　回顾性成组病例分析。结果　CSCS平均发生率为 1 7% ,占全部低钠血症的 (HN) 2 0 7% ;老年患者发生率高于非老年患者 ,也高于平均发生率 (P <0 .0 5 ) ;非老年患者中 ,男女CSCS发生率无差异 ,老年患者中 ,女性CSCS发生率高于男性 (P <0 .0 5 ) ;CSCS患者基础体重比非CSCS患者低 (P <0 .0 5 ) ,其中老年CSCS组与非老年CSCS组之间无差异 (P >0 .0 5 )。CSCS患者血清钠平均水平比非CSCS组低钠血症患者高 (P <0 .0 5 ) ,老年组CSCS与非老年CSCS组之间无差异 ;所有的CSCS患者血浆渗透压计算值低于正常 ;慢性消耗性疾病占本组CSCS病例的 80 0 % ,急性重症疾病占 2 0 .0 %。结论　CSCS并非罕见的临床综合征 ,老年患者尤其是老年女性、低体重患者易患CSCS ;CSCS患者血浆渗透压计算值正常或稍低于正常 ;CSCS多见于慢性消耗性疾病和急性重症疾病患者。Objective To investigate the clinical characters of chronic sick cell syndrome(CSCS) in the inpatients.Methods The retrospective study was adopted to analyze the discharged cases of CSCS.Results The average incidence of CSCS in all the inpatients included was 1.7%,and CSCS cases shared 20.7% of all clinical HN.The incidence of CSCS in the aged inpatients was higher than that of the non-aged,and than the average incidence (P<0.05).The incidence between male and female in the non-aged cases had no statistical difference average incidence (Fisher′s exact=0.485),but there was difference in the aged cases(P<0.05).The basic body weight of the CSCS patient was lower than that of the non-CSCS (P<0.05) , but there was no difference between the aged and non-aged groups(P>0.05).The level of SNa was higher in CSCS than that of general HN cases(P<0.05),but there was no difference between the aged and non-aged groups(P>0.05).The calculated value of plasma osmotic pressure of 46.7% cases with CSCS was lower than the normal value.The consumptive diseases accounted for 80.0% of CSCS, and the critical disease for 20.0%.Conclusions CSCS is not a rare clinical syndrome. The incidence is high in aged, especially in aged female patients.The individuals with low body weight are susceptible to CSCS.The calculated value of plasma osmotic pressure is normal or slightly lower.The CSCS frequently occurs in patients with consumptive or critical diseases.
