肠内免疫营养液对胃肠道恶性肿瘤病人术后机体的影响

摘　　要：[目的 ]探讨围手术期给予肠内免疫营养液对胃肠道恶性肿瘤手术病人机体免疫功能及并发症的影响。 [方法 ]将 60例胃肠道恶性肿瘤病人随机分为肠内免疫营养组(IN组 )和肠内普通营养组 (SN组 ) ,分别给予肠内免疫营养制剂及普通营养制剂 ,观察术前 5d及术后第 1天、第 4天和第 8天细胞和体液免疫指标、肝功能、术后并发症、肛门排便 /排气时间等变化。[结果 ]IN组术后第 4天和第 8天时IgG、IgM、CD4、NK细胞水平明显高于SN组 ;术后第 8天血清前白蛋白和白蛋白水平IN组高于SN组 (P <0 .0 5 ) ,而术后并发症发生率IN组 (3 0 .0 % )低于SN组(5 6.7% ) ,肛门排便 /排气时间两组无差异。 [结论 ]术前及术后给予肠内免疫营养支持 ,可提高胃肠道恶性肿瘤病人术后免疫力 ,减少并发症 ,提升血清前白蛋白和白蛋白水平。Objective:to probe into the effect of enteral immune nutrition fluids on immune functions and complications of patients with malignant gastrointestinal tract tumor. Method: a total of 60 patients with malignant tumor of gastrointestinal tract were selected and divided into enteral immunization nutrient group (IN) and enteral common nutrient group (SN) randomly, and were given enteral immunization nutrient fluids and enteral common nutritive fluids respectively. Then items including cellular immunity, humoral immunity, liver function, postoperative complications, defecation and anal exhaust time of all patients were monitored on five days before operation, the first, fourth and eighth days after operation. Result: the IgG, IgM,CD 4 and NK cells levels of patients in IN group were much higher than that in SN group on the fourth and eighth days after operation. The human serum prealbumin and albumen levels of IN group patients were much higher than that of SN group on the eighth day after operation (P<0.05).The incidence of postoperative complications of patients in IN (30.0%) were lower than that of patients in SN group (56.7%).There was no difference in terms of defecation time and anal exhaust time between the two group patients. Conclusion: it showed that to perform enteral immunization nutrient supports for patients with malignant tumor of gastrointestinal tract before and after operation could enhance their postoperative immunity, reduce the complications and raise human serum prealbumin and albumen of them.
