摘 要:目的:研究抑肽酶(aprotinin)在犬肺移植手术中对 肺组织缺血/再灌注损伤的作用.方法:20只成年雄性犬随机 分为两组,每组供体和受体各5只.建立肺移植动物模型.I组 (抑肽酶组):术中受体给予aprotinin静滴(25u/kg),II组 (对照组):给予等容积50g/L葡萄糖溶液.恢复血流灌注1h 后,取移植肺组织为标本,检测肺组织中丙二醛(MDA)含量、 髓过氧化物酶(MPO)活力、一氧化氮(NO)含量、肺组织含水 量并做HE染色光镜下观察病理变化.结果:aprotinin组的肺 组织含水量(73.65±4.31)%较对照组(81.33±3.67)%显著 降低(P<0.05).aprotinin组肺组织中髓过氧化物酶活性 (0.21±0.05ΔOD/min·g)显著(P<0.05)低于对照组 (0.37±0.10ΔOD/min·g).抑肽酶组丙二醛每毫克蛋白含 量(0.17±0.004)nmol显著低于对照组(0.29±0.01)nmol. aprotinin组一氧化氮每毫克蛋白含量(12.60±8.AIM: To evaluate the effect of aprotinin on the ischemia/reperfusion injury of the lung in lung transplantation in canines. METHODS: Twenty male canines were randomly divided into two groups. In each group the canines were randomly assigned to donors (n=5) and recipients (n=5). The recipients received either aprotinin 25 U/kg iv. infusion (Group I) or equivalent volumes of 50 g/L glucose i.v.(Group II) before reperfusion. The lung tissue samples were obtained one hour after reperfusion. The pulmonary water content, tissue myeloperoxidase (MPO) activity, content of MDA and nitric oxide (NO) were measured. Microscopic examination of lungs was also conducted. RESULTS: The pulmonary water content (73.65±4.31)%, the tissue MPO activity (0.21± 0.05 ΔOD/min·g) , the content of MDA (0.17±0.004 nmol/mg·prot) and the content of NO (12.60±8.73 nmol/mg·prot) in group I were significantly lower respectively than those in control (81.33±3.67)%, (0.37±0.10 ΔOD/min·g), (0.29±0.010 nmol/mg·prot) and (35.09±13.73 nmol/g·prot) (P<0.05). The microscopic examination showed that there were severe leukocyte infiltration and edema formation in the alveolar space in group II, but the changes were less severe in group I. CONCLUSION: The application of aprotinin before reperfusion can protect the lung against ischemia/reperfusion injury in lung transplantation in canine.
