免钛夹法腹腔镜胆囊切除术应用800例

摘　　要：目的　探讨腹腔镜胆囊切除术中不使用钛夹的技术 ,观察其临床效果。方法　 80 0例腹腔镜胆囊切除术术中采用镜下打结、线圈套扎及超声刀技术处理胆囊动脉或胆囊管完成手术 ;对照组采用钛夹夹闭胆囊动脉与胆囊管完成腹腔镜胆囊切除术 32 6例 ,比较临床效果。结果　 80 0例全部顺利完成 ,无中转开腹。免钛夹组手术时间较长 (P <0 0 1) ,术后肠道功能恢复时间、术后住院时间等恢复指标两组类似 ,两组均无术后大出血、胆瘘等并发症发生。结论　借助镜下打结、线圈套扎及超声刀技术 ,腹腔镜胆囊切除术可以不使用钛夹 ,手术安全可靠。Objective To explore the effect of laparoscopic cholecystectomy without using titanium-clips. Methods 800 patients(Group 1) underwent laparoscopic cholecystectomy without using titanium-clips were compared to 326 patients(Group 2) using titanium-clips. Results All cases were successfully operated without conversion to laparotomy. The operating time was longer in Group 1(P<0.01). The recovery time of postoperative intestine function and the length of postoperative hospital stay were similar, and there were no bile leakage in both groups. Conclusion Laparoscopic cholecystectomy without titanium-clips is safe.
