外阴白斑和外阴鳞癌中基底膜蛋白的研究

摘　　要：目的 :　研究层黏连蛋白 (LN)和IV型胶原在外阴白斑和外阴鳞癌中的表达。方法 :　采用SABC免疫组化法 ,对 5 9例外阴白斑和 9例外阴鳞癌标本进行基底膜蛋白染色。结果 :　正常皮肤组织、外阴白斑 (增生型、硬化萎缩型和混合型 )LN和IV型胶原基底膜染色密度高 ,呈连续线状。LN和IV型胶原的染色密度和基底膜的连续性与外阴白斑的异常增生度及鳞癌的分化程度 (由高分化到低分化 )呈负相关。结论 :　基底膜蛋白的表达和分布与外阴白斑分型有关 ,外阴鳞癌和不典型增生组织中表达阳性率明显低于外阴白斑各型 。Objective: To determine the expression of laminin (LN) and type IV collagen in patients with leukoplakia and squamous cell carcinoma of vulvae. Methods: The expression of LN and type IV collagen in specimens from 59 cases of vulvar leukoplakia and 9 cases of squamous cell carcinoma was measured by immunohistochemical method. Results: The staining pattern of basement membrane in normal skin tissue, vulvar leukoplakia (hyperplasia type, sclerotic atrophic type and mixed type) with collagen and laminin antibodies was continuous and the staining showed high intensity. The expression of collagen IV and LN demonstrated that the staining intensity and basement membrane continuity were negatively correlated to the dysplastic extent of vulvar leukoplakia and differential status of tumor. Conclusion: The expression of basement membrane protein is dependent on the types of vulvar leukoplakia. The positivity rate of expression of basement membrane protein is lower in squamous cell carcinoma and atypical hyperplasia than that in vulvar leukoplakia. The value of expression of basement membrane protein may be used as a biological marker in early assessing vulvar precancerous lesions.
