摘  要:目的 探讨L -卡尼汀对大鼠心脏移植再灌注损伤的保护作用。方法 采用大鼠颈部异位心脏移植模型 ,随机分为2组 :L -卡尼汀组 ,对照组。再灌注 2h测定心肌脂质过氧化终产物MDA、过氧化物歧化酶SOD的含量 ,电镜观察心肌组织的病理变化。TUNEL法染色检测细胞凋亡 ,以心肌凋亡阳性细胞数占总心肌细胞数的百分比作为心肌细胞凋亡指数。结果 L -卡尼汀组心肌线粒体等结构损伤明显减轻 ,心肌MDA明显减少 (P <0 0 1) ,SOD明显增加 (P <0 0 1) ,心肌细胞凋亡指数明显降低 [(7 14± 0 3 8) %vs (3 8 3 6± 3 16) % ,P <0 0 1) ]。结论 L -卡尼汀能有效减轻脂质过氧化反应 ,保护心肌线粒体 ,抑制心肌细胞凋亡 ,对心脏移植再灌注损伤有显著保护作用。Objective To explore the protective effect of L-carnitine on the reperfusion injury of rat heart transplantation. Methods The rat model of cervical heterotopic heart transplantation was set up. L-carnitine was injected to acceptor 4h before transplantation. Myocardiac malondialdehyde(MDA) and superoxide dismutase(SOD) concentrations were measured 2h after reperfusion. Pathological change of myocardium, especially mitochondria, was detected by transmission electronic microscope. Apoptosis of myocardial cells was determinded by TUNEL. Results Myocardial MDA concentration in L-carnitine treatment group was significantly lower than that in control group (P<0.01), while SOD concentration was significantly higher than that in control group (P<0.01). The injuries of myocardial ultrastructure, especially mitochondrion, in L-carnitine treatment group was significantly less than those in control group, and apoptotic index of mycardial cells was also significately lower than that in control group. Conclusion L-carnitine can decrease lipidoxidation reation, abate the pathological lesions of mitochondrion, and inhibit apoptosis in the myocardium of rat transplant heart. L-carnitine can protect rat transplant heart against reperfusion injury.
