摘  要:目的 探讨 β -七叶皂甙钠对单侧睾丸扭转 /复位后双侧睾丸细胞凋亡及相关基因蛋白表达的影响。 方法  3 0只青春期Wistar大鼠随机等分为对照组 (C组 )、扭转 /复位模型组 (M组 ) (左侧睾丸扭转 72 0°、2h后复位 )和 β -七叶皂甙钠治疗组 (T组 )。术后 48h获取双侧睾丸 ,分别以原位缺口末端标记法 (TUNEL)及免疫组织化学法检测凋亡细胞和bcl -2、Bax基因蛋白表达。结果 与C组比较 ,M组双侧睾丸生精细胞凋亡指数明显增加、bcl -2表达下降和Bax表达增强 (P <0 0 1) ,bcl -2 /Bax比值降低。与M组比较 ,T组双侧睾丸生精细胞凋亡指数明显减少 (P <0 0 1)、病理损害减轻 ,Bax低表达 (P <0 0 1) ,bcl -2 /Bax比值增高。结论 单侧睾丸扭转复位后 ,双侧睾丸生精细胞凋亡加剧 ,bax、bcl -2参与了细胞凋亡的调控 ;β -七叶皂甙钠通过抑制bax的表达 ,提高bcl -2 /Bax比值 ,逆转生精细胞凋亡增加而发挥保护作用。Objective To investigate the effects of β-aescin on the germ cell apoptosis and expression of bcl-2 and Bax proteins in bilateral testes following ipsilateral testicular torsion and detorsion. Methods Thirty adolescent male Wistar rats were randomly divided into control group (C group), model group of testicular torsion and detorsion (M group) and β-aescin treatment group (T group), each group containing 10 animals. In M and T groups, rats underwent a clockwise 720° ipsilateral testicular torsion for 2 h followed by detorsion, and the rats in T group were intraperitoneally injected with β-aescin 15 min before detorsion. 48h after operation, bilateral testes were collected, and used to detect cell apoptosis and expressions of bcl-2 and Bax by TUNEL technique and immunohistochemical method, respectively. Results In M group, the apoptotic index of the germ cells and Bax expression of bilateral testes significantly increased compared with C group (P<0.01), while bcl-2 expression obviously decreased (P<0.01). In T group, the apoptotic index and Bax expression of the bilateral testes significantly decreased(P<0.01), the ratio of bcl-2/Bax raised, and the pathological lesions of the bilateral testicles were obviously alleviated compared with M group. Conclusion The apoptosis of germ cell in bilateral testes significantly increased due to ipsilateral testicular torsion and detorsion, which was closely associated with the expression of bcl-2 and Bax in bilateral testes. β-aescin was effective in the treatment of ipsilateral testicular torsion and detorsion by increasing bcl-2/Bax ratio and decreasing the apoptosis of germ cells in the bilateral testes.
