摘 要:目的:以葡萄糖转移实验检测肿胀液灌注压、注射针管内径,利多卡因、肾上腺素、庆大霉素对脂肪颗粒活性的确切影响,为脂肪移植术中如何控制这些因素提供参考。方法:于同一患者下腹部左右侧分别灌注 1∶1 和 1∶3 的肿胀液,同条件抽取,获两组样本进行葡萄糖转移实验,t 检验比较 2 组样本的葡萄糖转移量即活性有无差异。用侧孔为 2.0mm×5mm 的吸管抽取的脂肪颗粒分 3 组,分别经注射器乳头(内径 2mm)、16 号针头(内径 1.5mm)、12 号针头(内径 1.1mm)注射于培养皿中进行葡萄糖转移实验,方差分析检验比较 3 组样本的葡萄糖转移量即活性有无差异。将同条件获取的脂肪颗粒分 4 组,分别置于含0.08%利多卡因、0.0002%(1/50 万)肾上腺素、1kU/ml 庆大霉素及常规 DMEM(对照组)中进行葡萄糖转移实验,方差分析检验比较前 3 组样本的葡萄糖转移量即活性与对照组有无差异。结果:1∶1 和 1∶3 灌注压下获取的 2 组脂肪颗粒葡萄糖转移量差异不显著(P>0.05);脂肪颗粒的葡萄糖转移量随注射时所经注射器乳头或针管的内径变小而依次降低,差异显著(P<0.05);在含 0.08%利多卡因、0.0002%(1/50 万)肾上腺素、1kU/ml 庆大霉素的 DMEM 中进行孵育的脂肪颗粒的葡萄糖转移量均较对照组显著降低(P<0.05)。Objective We examined the definitive effect of tumescent perfusion pressure, diameter of fat injection needle, lidocaine, epinephrine and garamycin on fat viability by glucose transportation test to offer reference to clinical practice. Methods We infiltrated tumescent in a ratio of roughly 1:1 (1 cc of tumescent per cubic centimeter of fat to be harvested) in left lower abdomen of one patient and 1:3 in right lower abdomen of same patient and harvested two groups of fat. We examined the glucose transportation quantities of the two groups of fat by glucose transportation test and analyzed the result. We injected fat aspirated with the cannula with 2.0mm×5mm distal opening into dishes respectively through syringe nipple, 16 gauge needle and 12 gauge needle,and then put them into glucose transportation test. The glucose transportation quantities of the three groups were compared. Four groups of fat harvested under same conditions wers put into DMEM (control group), DMEM containing 0.08% lidocaine, DMEM containing 0.0002% epinephrine and DMEM containing 1ku/ml garamycin (four experimental groups) respectively for incubating and compared their glucose transportation quantities. Results The glucose transportation quantities of fat harvested under different tumescent perfusion pressure were same (P>0.05). The glucose transportation quantities of fat injected through syringe nipple, 16 gauge needle and 12 gauge needle significantly decreased in turn (P<0.05). The glucose transportation quantities of fat incubated respectively in DMEM containing 0.08% lidocaine, DMEM containing 0.0002% epinephrine and DMEM containing 1ku/ml garamycin were all lower than control group (P<0.05). Conclusion The tumescent perfusion pressure in the scope of clinical application didn′t affect fat viability. The needle diameter smaller than that of fat particles can damage fat viability. Lidocaine, epinephrine and garamycin all can inhibit the viability of fat.
