两种入路眼袋整复术中发生的眼心反射

摘　　要：目的:通过眼袋整复术观察眼心反射,并探讨其可能的发生机制及预防措施。方法:共对 80 例受术者进行眼袋整复,其中经结膜入路 42 例,经皮肤入路 38 例,术中进行动态心电图监测,分析比较术前、术中心率的变化。结果:眼心反射阳性标准:心率较术前每分钟减慢 10 次以上。所有病人中,共 13 例出现眼心反射,其中经结膜入路 8 例,经皮肤入路 5 例,均在提拉剪除眶隔脂肪时发生。眼心反射发生率为 16.25%。 结论:眼袋整复术中可以发生眼心反射,即出现心率减慢、心脏节律的改变。所以手术应轻柔、准确、快捷,以尽量避免眼心反射的发生。Objective To observe the oculocardiac reflex during the blepharoplasty and study its possible mechanism and counterpart defence measures. Methods A total of 42 cases through conjunctival incision and 38 cases through skin incision were studied by dynamic electrocardiography to analyst the changing of heart rate before and after operations. Results Heart rate decreasing by over 10 times during operations is regarded as the positive standard of the oculocardiac reflex. The oculocardiac reflex appeared in 13 cases of all cases (8cases through conjunctival incision and 5 cases through skin incision). The OCR was observated during resecting the orbital fats.The incidence of oculocardiac reflex was 16.25%. Conclusion The reflex can occur during blepharoplasty and it may result in heart rate decreasing and heart rhythm altering. We should operate gentlely and accurately,also avoid overly stress on eyeball to elicit the oculocardiac reflex.
