氯沙坦和拜心同联合治疗对高血压病患者肾功能的影响

摘　　要：目的　观察氯沙坦、拜心同单独治疗和联合治疗对 1、2级高血压病患者的降压效果及对肾功能的影响。方法　 15 0例 1、2级高血压病患者随机分成 3组 :氯沙坦组 (5 0mgqd ,5 0例 ) ;拜心同组 (30mgqd ,5 0例 ) ;氯沙坦和拜心同联合治疗组 (氯沙坦 5 0mgqd ;拜心同 30mgqd ,5 0例 )。疗程 4周 ,治疗前后观察血压及肾功能。结果　①3组患者治疗后均能显著降低血压 (P <0 .0 1) ,组间降压有效率及降压幅度无显著性差异 (P >0 .0 5 )。② 3组患者治疗后均能显著降低 2 4h尿白蛋白、2 4h尿蛋白及血、尿 β2微球蛋白 (P <0 .0 1) ,单独治疗组间差异无显著性 (P>0 .0 5 ) ,联合治疗组降低尿蛋白排泄的幅度比单独治疗组高 (P <0 .0 1) ,③氯沙坦组与联合治疗组治疗后均能显著降低血尿酸 (P <0 .0 1)。结论　氯沙坦 ,拜心同单独治疗及联合治疗均可显著降低血压及减少蛋白尿的排泄 。Objective TO examine the effect of losartan and a da lat used individually or in combination on blood pressure and renal function in patients with grade 1 or 2 of essential hypertension (EH).Methods O ne hundred fifty patients with grade 1 or 2 EH were divided randomly into 3 grou ps: losartan (50 mg qd,n=50)、adalat(30 mg qd n=50)and combined treatmen t(losartan 50 mg qd + adalat 30 mg qd,n=50).Treatment lasted for 4 weeks.Blo od pressure and renal function were measured before and after the treatments.Results ①After treatment,blood pressure was significantly decreased i n all the three groups(P<0.01),but no significant difference was found in th e three groups.②After treatment, urinary albumin excretions were significantly decreased(P<0.01).Magnitude of decrease of urinary albumin excretions in the combined treatment group was higher than those in the losartan and adalat group s(P<0.01).No significant difference was found between losartan and adalat gr oups(P>0.05).After treatment, blood uric acid was significantly decreased in losartan and combined treatment groups(P<0.05).③After treatment, blood ur ic acid was significantly decreased in losartan and combined treatment groups( P<0.01).Conclusion Losartan adalat and combined treatment can all significantly decease blood pressure and urinary albumin excretion,but combined treatment has additive effect on the protection of renal function.
