长春瑞滨外漏组织损伤后局部封闭的实验研究

摘　　要：目的探讨长春瑞滨静脉注射外渗时采用不同药物局部封闭对损伤组织的保护作用。方法将 19只健康新西兰兔制成局部长春瑞滨损伤模型 ,其左侧大腿定为实验组 ,右侧为对照组。实验组用 1%利多卡因局部封闭 ,对照组用 0 .9%氯化钠注射液封闭。于封闭后 2 4、72、16 8、336h分别行肉眼观察和镜下观察。结果肉眼观察 ,局部组织红肿、溃疡 2 4、72、16 8h实验组与对照组比较 ,差异无显著性意义 (均P >0 .0 5 ) ,而 336h时差异有显著性意义(P <0 .0 5 )。镜下观察 ,对照组局部组织嗜中性粒细胞及嗜酸性细胞浸润明显 ,炎症反应严重 ;淋巴细胞浸润少见 ;实验组局部组织嗜中性粒细胞及嗜酸性细胞浸润不明显 ,炎症反应较轻 ,可见淋巴细胞浸润。结论长春瑞滨静脉用药外渗时采用利多卡因局部封闭可减轻组织损伤。Objective To research the protective effect of Lidocain in local blockade on tissue injury caused by extravasation of Vinorelbine. Methods Nineteen healthy New Zealand rabbits were used to make models of tissue injury by Vinorelbine. The left tight was used as experimental side and right tight as control. The experimental side was blocked with 1% Lidocain and the control side with 0.9% normal saline. The blocked sides were observed by unaided eyes and under microscope 24 h, 72 h, 168 h and 336 h after the blockade. Results There was no significant differences in red swelling of local tissues and ulcer between the experimental sides and control sides 24 h, 72 h, and 168 h after the blockade as showed by the naked-eye examination (all P>0.05). 336 h after the blockade, significant differences were in these changes (P<0.05). Microscopically, in control sides, conspicuous infiltration of eosinophilic leucocytes, neutrophil leucocytes and monocytes were found in the local tissues, the inflammation was severe and infiltration of lymphocytes were rarely seen. In the experimental sides, the infiltration of eosinophilic leucocytes, neutrophil leucocytes and monocytes were not obvious, inflamation reaction was mild and infiltration of lymphocytes could be found.Conclusion Lidocaine can be used for minimizing the injury caused by the extravasation of Vinorelbine in blockade.
