超声心动图在心内膜垫缺损诊断中的应用

摘　　要：目的　评价超声心动图对心内膜垫缺损 (ECD)的诊断价值并分析部分合并畸形漏诊的原因 ;方法　对 33例患者的超声心动图结果进行回顾性分析 ,并与部分手术病例结果进行对照 ;结果　 33例中完全型ECD 7例 ,其中手术 2例 ,部分型 ECD 2 6例 ,其中手术 2 0例。合并畸形有卵圆孔未闭、 II孔型房间隔缺损(ASD)、单心房、室间隔膜部瘤等 ;结论　超声心动图能较好地显示 ECD的解剖畸形和复杂的血流动力学情况 ,并能检出大部分合并的畸形。Objective To evaluate the value of ultrasound in endocardial cushion defect(ECD)diagnosis and to analyse the misdiagnosis reason of some cases.Methods Ultrasound features were retrospectively analysed in 33 cases of ECD,and compared to the operating results of some cases.Results Among 33 cases,7 cases were with complete ECD(CECD)(2 cases were operated) and 26 cases were with partial ECD(PECD)(20 cases were operated).Accompanied deformations included patent fossa ovalis,secundum atrial septal defect,single atrium and aneurysm of membranous interventricular septum etc.Conclusions Ultrasound can clearly demonstrate all anatomic deformations and complicated hemodynamic changes of ECD,as well as most of other coexistent deformities.
