具有自动夺获功能的心室起搏器临床应用

摘　　要：目的:评定15例心室起搏器的自动夺获功能的功效和安全性。方法:15例患者,男性9例,女性6例,年龄37～81岁,均有器质性心脏病,基础心律:10例为慢性心房颤动伴 RR 长间歇,4例病态窦房结综合征,其中2例合并阵发性心房颤动,1例三度房室阻滞。11例植入 Regency SC+型 VVI 起搏器,4例植入 Regency SR+型 VVIR 起搏器。结果:起搏器植入后随访1～13个月,起搏器工作良好,QRS(R)振幅9.65±2.55 mV,ER 振幅7.85±3.87 mV,两者相关系数(r)为0.15(n=55)。应用 VARIO方法和自动夺获法测定的起搏阈值均在0.3～1.5 V 之间,两者相关系数为0.99(n=46)。极化电位低于1 mV 者占全部测定例次的24%,高于1 mV 的极化电位幅度为1.38±0.13 mV(n=37)。结论:与普通 VVI(R)起搏器相比,具有自动夺获功能的 VVI(R)起搏器功耗低,预计使用寿命长,起搏安全可靠,随访省时。Objective:To assess the efficacy and safety of VVI or VVIR pacemakers with autocapture algorithm implanted in 15 patients each.Methods:Autocapture pacemakers were implanted in 15 pa- tients i9 male,6 female)with organic heart diseases.These patients ranged in age from 37 to 81 years.Ten of them bad atrial fibrillation with long pause,4 had sick sinus syndrome (2 had paroxysmal atrial fibrilla- tion),and one had complete atrioventricular block.11 patients had a Regency SC + VVI pacemaker im- planted,the other 4 patients had a Regency SR+VVIR pacemaker implanted (Pacesetter,St.Jude Medical Company).Results:All pacemakers were working well during the follow up from 1 to 13 months.The QRS amplitude was 9.65±2.55 mV ,the ER amplitude was 7.85±3.87 mV ,the coefficient of correlation was 0.15(n=55).Pacing threshold were among 0.3 to 1.5 Volts measuring with VARIO or autocapture threshold test,the coefficient of correlation was 0.99(n=46).Twenty-four percent polarized potentials were less than 1 mV,others had ,polarized potentials of 1.38±0.13 mV(n=37)Conclusion:Com- pared with ordinary ones,pacemakers with autocapture algorithm had the superiority of low energy con- suming,long life-span,high safety and time-saving on follow up.
