摘 要:目的 :应用99mTc MIBI心肌血流断层定量显像及冠状动脉造影研究增强型体外反搏 (EECP)对促冠脉侧支循环形成及改善心肌血供的价值。材料和方法 :12只闭胸式心肌梗塞模型犬均分为对照组和EECP治疗组 ,99mTc MIBI心肌断层定量显像在术后 4~ 6天及第 6周实验结束时进行 ,计算心肌缺损分数 (DF)及心肌挽救分数 (MSF =首次显像DF—后次显像DF)。结果 :EECP组心肌显像有 83 .3 % (5 /6)的实验犬治疗后心肌缺血改善 ,而对照组仅 1只 (16.6% )好转 ,且MSF值呈负值。冠脉造影与免疫组化染色分析表明EECP组冠脉侧枝开放数量及阳性染色区域面积也明显多于对照组。结论Purpose: To study the value of external counterpulsation on promoting coronary angiogenesis and improving myocardial blood perfusion by myocardial SPECT quantitation and coronary artery angiography. Materials and Methods: 12 canines with coronary occlusion by closed-chest model were divided into contrast group and enhanced external counterpulsation (EECP) group. All canines were performed 99m Tc MIBI myocardial SPECT 4-6 days after coronary occlusion and at the end of study. The myocardium defect fraction (DF) and saved fractions (MSF=first DF-post DF)were calculated. Results: Myocardial blood increased in 83.3%(5/6)experimental canines with EECP therapy,and in only 16.6%(1/6) of contraol group with negative MSF. The results also showed that the amount of coronay collaterals and positive stained area was significantly higher in EECP group than in contrast group by coronary artery angiography and immunohistchemical analysis. Conclusion: Results indicate EECP could increase coronay collaterals and decrease the infarct areas effectively.
